      目的 探讨实时超声造影在肝肿瘤诊断中的时相改变及其价值。方法 选用百胜Megas超声诊断仪和Bracco公司的SonoVue超声造影剂,对29例共34个肝内实质占位病变进行实时谐波超声造影检查,其中16个原发性肝癌,4个转移性肝癌,9个肝血管瘤,5个局灶性结节性增生。结果 经超声造影后,34个病灶均有不同程度的增强。肝癌的增强减退时间(57.6±15.7s)明显比其他良性肿瘤为早(P<0.05);肝血管瘤的增强峰值时间(65.0±24.6s)明显比其他肿瘤长(P<0.05)。如以肝肿瘤在动脉相增强而在门脉相减退呈低回声作为诊断肝癌的指标,则超声造影诊断肝癌的敏感性、特异性及准确性分别为95%(19/20)、100%(14/14)和97%(33/34)。结论 实时超声造影能反映不同肝肿瘤在不同时相上的不同表现,可帮助提高肝肿瘤超声诊断的准确性。

      Objective To evaluate the value of real time enhanced phase- ultrasonograpgy in diagnosis and differential diagnosis of hepatic tumor.Methods Thirty-four hepatic solid lesions in 29 patients were examined by real time enhanced-phase- ultrasonography, including 16 primary malignant tumors, 4 metastatic tumors, 5 focal nodular hyperplasias, and 9 hemangiomas in the liver. All cases were confirmed by histopathology. The ultrasound unit (Esaote Megas) with real time contrast-enhanced technique was applied. SonoVue (Bracco, Italy) was used as contrast agent. Results After injecting SonoVue, all lesions were variously enhanced on real-time ultrasonogram. The time of reducing enhancement (57.6±15.7s) in hepatic malignant tumors was obviously earlier than that in hepatic benign tumors (P<0.05). The time of peak enhancement (65.0±24.6s) in hemangiomas was obviously longer than that in other hepatic tumors (P<0.05). The lesions enhanced in arterial phase and decreased in portal phase with administration of contrast agent were considered as an indicator for malignant tumor. The sensitivity, specificity and accuracy in the diagnosis of hepatic carcinoma with real-time contrast ultrasonography were 95%(19/20), 100%(14/14) and 97%(33/34) respectively. Conclusion Based on various pathology, the enhancement of hepatic lesions appeared in different circulation phase of the liver on real time enhanced phase- ultrasonography, which will improve the accuracy of the diagnosis and differential diagnosis of liver neoplasm with ultrasonography.
