      目的 采用全方向M型超声心动图测定左室各节段室壁的收缩、舒张功能参数,了解正常人左心室上、中、下三个节段不同室壁的短轴方向局部运动状况,作为进一步研究的正常对照。方法 国产LEJ-1全方向M型心动图系统,对胸骨旁左室短轴二尖瓣水平、乳头肌水平、心尖水平的二维图像进行采集和定标,用手动分析法对左室16个室壁节段的运动曲线进行测量,测量参数包括:收缩期最大运动幅度(R),收缩末期室壁厚度(Ts),舒张末期室壁厚度(Td),舒张三分之一时室壁厚度(T1/3d),心内膜舒张早期运动速度(Vd)与收缩期最大运动速度(Vs),以及心电图R波顶点到室壁舒张开始的时间间隔(R-d),每种测定取连续三次测量的平均值,并计算室壁增厚率(△T%)、舒张早期运动速度与收缩期最大运动速度的比值(Vd/Vs)及舒张三分之一时室壁变薄率(△T1/3d%),以及16节段室壁R-d的最大差值R-D。结果 左室各节段室壁的局部功能指标平均测值:R为(8.97±1.5)mm,△T%为(56.47±8.2)%,Vs为(38.07±9.0)cm/s,Vd为(47.67±13.9)cm/s,Vd/Vs为1.25±0.17,△T1/3d%为(64.84±6.27)%。总体看,后壁各项指标测值较大,而室间隔的较小;中、上段室壁的收缩和舒张运动较心尖段强。结论 全方向M型超声心动图可以对所有节段室壁的局部运动功能进行定量检测,正常左室各节段室壁的收缩和舒张功能各节段间正常值存在一定差异。

      Objective We measured the regional wall systolic and diastolic indexes on normal subjectives by omni-directional M-mode echocardiography to assess the left ventricular regional wall function and build a control group. Methods The study of short-axis sections at mitral valve level, papillary muscle level and apical level were recorded and analyzed by LEJ-1 omni-directional M-mode echocardiography system developed. The left ventricle was subdivided into 16 segments of three short-axis sections with wall range during systolic period (R), end-systolic thickness (Ts), end-diastolic thickness (Td), thickness at one-third relaxation period (T1/3d), systolic velocities (Vs), early-diastolic velocities (Vd) and time from ECG R wave to the beginning of diastole (R-d) of segmental walls were measured, and the percentage of wall thickening(△T%), the Vd/Vs ratio, the percentage of wall thinning in one-third relaxation period (△T1/3d%) of each wall were measured. The coefficient of variation of the R-d time (R-D) was calculated to determine the synchronicity of the regional beginning of diastole. Results The respective average values of all regional functional indexes in total segments were (8.97±1.5)mm (R), (56.47±8.2)% (△T%), (38.07±9.0)cm/s (Vs), (47.67±13.9)cm/s (Vd), (1.25±0.17) (Vd/Vs), (64.84±6.27)% (△T1/3d%).The values of posterior wall were more than those of other walls, the values of interventricular septum were less than those of other walls; the systolic and diastolic motions of basic and middle segments stronger than that of apical ones. Conclusion Omni-directional M-mode echocardiography was capable of detecting all segments of left ventricle quantitatively, and there were differences between normal ranges of the values of regional systolic and diastolic functional indexes.
