| 目的 探讨嗜酸性胃肠炎的CT诊断价值。方法 分析5例嗜酸性胃肠炎的CT及临床、病理表现特点,并回顾复习文献报道所见。结果 临床主要表现有腹痛5例,外周血嗜酸粒细胞增高4例。CT上,5例均见有异常的胃肠道病变,主要有:胃肠道壁增厚5例,其中伴见分层状壁增厚3例,胃肠腔狭窄3例,黏膜皱襞粗大2例,多发溃疡、不全小肠梗阻、胃肠分泌液增多各1例。肿块4例,其中肠外肿块3例,肠内肿块1例;4例中2例呈均质密度伴强化,2例呈环形壁强化伴中心坏死液化。微少量腹水2例。结论 嗜酸性胃肠炎CT表现与嗜酸细胞浸润胃肠壁深度与广度有关;而分层状胃肠道壁增厚、伴黏膜皱襞粗大的壁增厚、坏死性或均质性肉芽肿具有一定的特征性,CT典型表现结合外周血嗜酸性细胞增多及过敏史的临床表现可提示本病的诊断。 |

| Objective To evaluate CT diagnostic value of eosinophilic gastroenteritis(EG).Methods The CT, clinical and pathological manifestations of 5 cases of EG were analyzed, and with a review of literatures.Results The main clinical features: 5 cases with abdominal pain, 4 with peripheral eosinophilia.There are abnormal findings of gastrointestinal tract in all 5 cases by CT, and the main CT findings were: wall-thickening in 5 cases, in which 3 cases with halo sign and stenosis, 2 with fold thickening sign, 1 with multiple ulcer or obstruction or hypersecretion; masses in 4 cases, in which 3 with outside and 1 with inside gastroenteritional cavity, 2 with homogenic soft-tissue density, and another with circular enhancement accompanyed by central liquefaction or necrosis; 2 cases with ascites.Conclusion CT features of EG are related to the location and layer of eosinophilic infiltration; gastroenteritional wall-thickening with halo sign and/or fold thickening,eosinophilic granuloma with circular enhancement or homogenic soft-tissue density might have some characteristics,and CT typical features combined with the clinical findings of peripheral eosinophilia and allergic history might suggest the diagnoses of the disease. |
