目的 采用双导微波发射,增大肝内凝固坏死范围,为临床一次性完全覆盖凝固较大肿瘤提供依据。方法 用离体新鲜猪肝,选择三种作用条件(30 W,10 min;35 W,10 min;40 W,10 min),在超声导向下将两根微波天线按不同间距同深度植入肝脏,采用同时和分时发射微波,术后剖视固化灶大小、形态。结果 同时双导在35 W,10 min 两针距1.5 cm时,凝固灶范围为5.8 cm×5.2 cm×5.0 cm,形态呈类球形,分时双导在同等条件下所形成凝固灶范围为5.5 cm×4.6 cm×4.2 cm,形态呈蝶形,融合欠佳。双导两针中间较两针外侧相对应处温度存在明显温度差,且升温迅速,高温持续时间长。结论 双导微波发射,凝固坏死范围明显增大,为临床治疗较大肝肿瘤提供了支持;同时双导凝固效果好于分时双导;双导微波发射所致热场不是两个孤立热场的简单线性叠加。 |

Objective To enlarge the scope of microwave coagulation in liver through double implanted electrodes radiation in order to completely coagulate larger tumor at one stroke.Methods Fresh pig livers in vitro,with the same depth and the different distance,two electrodes were implanted in the liver conducted by ultrasound.Microwave was transplanted in the liver at the same and different time at three kinds of power output (30 W,10 min;35 W,10 min;40 W,10 min).Dissecting the liver and measuring the coagulation area were done after treatment.Results Coagulated area was 5.8 cm×5.2 cm×5.0 cm when the distance of 1.5 cm between the two electrodes at power output of 35 W 10min simultaneously, its shape looked like a globe.Coagulated area was 5.5 cm×4.6 cm×4.2 cm when the distance of 1.5 cm between two electrodes at power output of 35 W 10min asynchronously, its shape looked like a butterfly.Conclusion A larger coagulative necrosis area was induced through double implanted microwave radiation.It provides a technical basis for treating larger masses of cancer.Coagulation effect conducted synchronously was superior to that conducted asynchronously.The thermal field induced by the double electrodes was not equal to two isolated fields that were simply overlapped in linerity. |
