目的 评价阿尔茲海默病(AD)患者与正常老年人脑结构磁化转移率的差异,并探讨其临床实用性。方法 对20例AD患者和20名健康老年人的海马结构、海马旁回、梭状回、丘脑和胼胝体的磁化转移率(MTR)进行测量,应用统计学方法检验AD患者与对照组间各脑结构MTR的差异,评价各结构测量值诊断AD的敏感性和特异性。结果 AD患者海马结构和海马旁回的MTR均较对照组显著下降(t海马=2.818,t海马旁回=1.096,P<0.05),而梭状回、胼胝体及丘脑的MTR虽较对照组略有减低,但差异均无显著性意义(t=0.409~0.565,P>0.05)。海马的MTR判别AD患者与正常者的敏感性、特异性和准确性分别为75%、85%和80%,高于海马旁回。结论 海马MTR 鉴别AD与正常人的敏感性和特异性均较高,而且操作简单、快速,可作为AD诊断的一项影像学指标。 |

Objective To assess whether regional magnetization transfer ratio (MTR) changes are present in Alzheimer's disease (AD) patients and to evaluate their clinical value.Methods The study group consisted of 20 patients with probable AD and 20 healthy control subjects.Magnetization transfer ratios of hippocampal formation (HF),paracampal gyrus (PHG),fusiform gyrus,corpus callosum and thalamus were measured.Statistical analysis was used to determine the ability of MTR in discriminating between AD patients and control subjects.Results MTRs in the hippocampal formation and parahippocampal gyrus were significantly lower in patients with AD than those in the control subjects (tHF=2.818,tPHG=1.096,P<0.05);however,MTRs of fusiform gyrus,corpus callosum and thalamus were not significantly different between AD group and control subjects.With discriminant analysis,the sensitivity, specificity and accuracy of the hippocampus were 75%,85%,and 80%,respectively, which were higher than those of the parahippocampal gyrus.Conclusion MTR in the hippocampus has higher sensitivity and specificity in its discriminative power between AD patients and control subjects.As a neuroimaging indicator,MTR measurements are practical for routine clinical use because the method is easier and faster. |
