| 目的 探讨血液和造血系统疾病的骨髓MRI表现和鉴别诊断。方法 经病理证实急性粒细胞白血病(AML)11例、急性淋巴细胞白血病(ALL)3例、慢性粒细胞白血病(CML)6例,骨髓纤维化(MF)6例,多发性骨髓瘤(MM)3例,再生障碍性贫血(AA)3例,行腰椎、股骨中上段骨髓SE及增强、脂肪抑制各序列成像MRI检查,与同期骨髓像白血病细胞计数(BMT)对比分析。结果 ①腰椎、股骨骨髓MRI像AML 11例中有9例、ALL 3例、MF 6例、MM3例、AA 2例T1WI和/或T2WI呈弥漫性斑片状低信号。②CML 6例T1WI、T2WI全部呈弥漫性低信号变化,Gd-DTPA增强效果明显。③ALL 3例、AA 1例股骨骨髓MR像中T1WI 、T2WI呈不规则形低信号。结论 MRI能为血液疾病的确诊和鉴别诊断提供影像学依据,并有助于临床治疗和疗效的评价。 |

| Objective To assess MRI value in differential diagnosis of hematological diseases. Methods In 32 cases, 11 cases were AML, 3 cases were ALL , 6 cases were CML, 6 cases were MF, 3 cases were MM and 3 cases were AA. Hematological MRI was obtained by the SE, fat-suppression and the contrast enhancement of vertebra, femur and correlated with BMT at the same time. Results MRI features included:①Diffuse spotty shadow: AML 9 cases, ALL 3 cases, MF 6 cases, MM 3 cases, AA 2 cases on T1WI and/or T2WI. ②Diffuse signal intensity shadow: CML 6 cases on both of T1WI and T2WI, but had contrast enhancement. ③Irregular shadow: ALL 3 cases, AA 1 cases on both of T1WI and T2WI of the femurs bone marrow. Conclusion The results suggested that knowledge of the MRI manifestation of hematological is useful for the diagnosis and management of patients. |
