      目的 探讨不同扫描参数对CT结肠造影检测结肠息肉的影响,寻找最适宜扫描参数,评价其临床应用价值。方法 ①取长约25cm 猪结肠10段和人结肠癌术后标本8段制成模拟息肉,应用不同参数进行扫描,采用不同的方法观察,分析图像质量。②12例患者共66个结肠息肉的CT原始图像资料传至工作站进行后处理,获取CT仿真结肠镜、多平面重建+轴位、表面遮盖显示+透明显示图像。所有病例均经手术病理和(或)电子结肠镜活检证实。结果 随准直和螺距的减小,图像质量提高,准直小于5.0mm,螺距小于2.0获得的图像较满意;肠段纵轴与扫描架平面成90°角时图像质量最好;轴位+MPR、CTC、SSD + Raysum对结肠息肉的检出率分别为65.15%、75.76%和51.52%。结论 不同的扫描参数对图像质量有很大影响,选择合适的参数准直宽度为5.0mm、螺距为1.5、肠腔内充盈气体,用多种方法观察图像可提高图像质量,提高息肉的检出率。

      Objective To investigate the influence of different scanning parameters on detecting colon polyps with CT colonoscopy (CTC) and to find out the optimal scanning parameters,as well as to evaluate the clinical application of CTC in the detection of polyps.Methods Three hundred and twenty-nine simulated polyps (3-12mm in size) were made using ten pig colons and eight postoperative specimens.The colon filled with gas underwent volum scanning.The images were evaluated with CTC,Axial + MPR,SSD + Raysum.The image quality was also assessed by three grades.Twenty-four data sets with 66 colon polyps were obtained from thin-section helical CT of the abdomen and pelvis in both the supine and prone positions in 12 patients.CTVC (CT virtual colonoscopy)、MPR (mulitiplanar reformation)+Axial and SSD (shaded surface display)+ Raysum images were generated by using different software in workstation.The results were compared with the findings of electrical colonoscopy or/and surgery and correlated with pathologic findings.Results The image quality of CT colonoscopy improved with the decrease of collimation and pitch.The optimal scanning parameters of artificial polyps were:collimation ≤5.0mm,pitch 1.0-2.0.The optimal angle of the colon lumen to the scanning gantry was 90°.The sensitivity for detecting colon polyps with MPR + Axial、CTVC and SSD + Raysum were 65.15%,75.76% and 51.52%.Conclusion The performance of CTVC has a close relationship with the scanning parameters.The optimal scanning parameters (5.0mm collimation,1.5 pitch) and the colon filled with gas satisfactorily,the combination of CTVC with other imaging methods,are all factors that contribute to improve the diagnostic sensitivity and accuracy for the detection of colon polyps.
