目的 探讨前列腺TURP术后改变及残存或再发病变的MRI诊断价值。方法 21例经手术病理或穿刺活检证实的TURP术后前列腺患者行MRI检查,5例加做直肠内线圈扫描和3D 1H MRSI检查,观察测量前列腺术后改变和前列腺残存和再发病变。并根据临床症状和尿动力学检查结果分为梗阻组14例和非梗阻组7例进行对照。结果 ①21例中19例可见前列腺中上段尿道增宽,梗阻组增宽尿道的长度和宽度较非梗阻组显著缩短(P<0.05~0.01),并可见明显的受压变形或移位改变;②MRI体线圈/盆腔线圈和直肠内线圈对TURP术后前列腺残存或再发病变诊断的准确率分别为69%和100%,3D 1H MRSI显示前列腺癌(胆碱+肌酸)/枸橼酸盐的比值明显高于正常前列腺和前列腺增生组织。结论 MRI可用于观察TURP术后前列腺和前列腺段尿道的变化,对术后残存或再发病变的诊断仍有较高的准确性。 |

Objective To discuss the value of MRI in the diagnosis of post-operative change and residual or recurrent disease of prostate after transurethral resection of the prostate (TURP). Methods 21 cases of post-TURP patients with prostate disease confirmed by histopathology of operation or biopsy were evaluated by MRI. 5 cases were evaluated by adding endorectal MRI and three dimensional 1H MR spectroscopic imaging (3D 1H MRSI). Changes of post-operative and the disease of prostate were observed and measured. Patients were divided into obstruction team (14 cases) and none obstruction team (7 cases) for comparing according to the clinical symptoms and the results of urine dynamic examination. Results ①Among 21 patients of post-TURP, the lumen in the upper and/or middle portion of the prostatic urethra was widened in 19 cases. The length and the width of the widened prostatic urethra of the obstruction team were statistically longer than that of none obstruction team (P<0.05-0.01) and the displacement or deformation caused by suppression and obstruction of prostatic residual or recurrent disease could be observed. ②The diagnostic accuracy of body/pelvic coil MRI and endorectal MRI for prostatic disease after TURP was 69% and 100%. The results of 3D 1H MRSI showed the ratio of (choline+creatine)/citrate of prostate cancer were notability higher than that of benign prostatic hyperplasia (BPH) and normal prostatic tissue. Conclusion MRI can be used to observe the related changes of prostate and the prostatic urethra after TURP. A relatively high accuracy can be achieved by using MRI to diagnose the residual or recurrent disease of prostate after TURP. |
