目的 进一步探讨组织多普勒成像(TDI)检测冠心病心肌缺血和存活的临床实用价值。方法应用TDI检测节段性室壁运动失常冠心病患者的左室前后壁心内膜运动速度(Vendo)、速度跨壁梯度(MVGm)及心肌中部心动周期中最大运动速度(TVC),并配合小剂量多巴酚丁胺负荷试验判断心肌活性。结果 梗塞部位Vendo、TVC及收缩期 MVGm均较非梗塞部位显著降低,有收缩储备部位Vendo、TVC及收缩期 MVGm均较无收缩储备部位显著增高。结论TDI对判断心肌缺血及预测再血管化后左室功能恢复有一定价值。 |

Objective To have further realization on the clinical value of tissue Doppler imaging (TDI) in myocardial ischemia and the viability in coronary artery disease. Methods On the short-axis views at papillary level, the velocity of endocardium (Vendo ) and epicardium (Vepi), the myocardial velocity gradient (MVGm) and the time velocity curve (TVC) were measured by TDI in both anterior wall and posterior wall of left ventricle. The myocardial viability was defined according to dobutamine-stimulating contractile response. Results The velocity of endocardium、myocardium and MVG were lower in infarct area than those in non-infarct area, but higher in the viable myocardium than those in the non-viable myocardium. Conclusion TDI is helpful to judge myocardial ischemia and predict myocardial function resume after revascularization. |
