目的 探讨超选择性子宫动脉造影的技术特点,了解子宫动脉形态特征,提高介入治疗的成功率。 方法 158例患者,年龄19~53岁,其中子宫肌瘤患者119例,宫外孕14例,盆腔恶性肿瘤15例,子宫大出血10例。造影导管选用Cobra、headhunter、MP、RUC导管等。 结果 两侧316支子宫动脉形态、直径、分布基本对称,196支起自臀下阴部动脉干,占62%;98支起自髂内动脉主干,占31%;13支起自臀上动脉,占4%;9支起点分辨不清。子宫动脉起始处管径比中间段细,与起始动脉之间的夹角变化大。171支子宫动脉与起始动脉的夹角≤60°,约占54%;136支夹角在60°和90°之间,约占43%;9支夹角≥90°,约占3%。 结论 45°斜位造影可帮助我们了解子宫动脉详细的分布情况,顺利完成操作;4.0~5.0F导管可完成超选择性子宫动脉造影的需要;决定插管成功的因素是认清子宫动脉起始的角度。 |

Objective To discuss the methods of superselective uterine arteriography and to improve the recognition of angiographical configuration of uterine arteries. Methods In 168 cases, there were 119 leiomyoma, 14 fallopian tubal pregnancy, 15 cervical carcinoma and 10 massive uterine haemorrhage. There were total 316 catheterization and angiography of uterine arteries.Via right or left thigh artery, angiography were studied with Cobra, headhunter, MP and RUC catheter. Results The shape, diameter and distribution of bilateral uterine artery were symmetrical. In 316 uterine arteries, there were 196 origirating from trunk of inferior gluteal and interanl pudendal arteries, 62%; 98 from main trunk of internal iliac artery, 31%; 13 form superior gluteal artery, 4%; the origin was faint of 9 branches, 3%. The uterine artery was divided into main trunk and distal segment, the main trunk was divided into initiative and middle segments. The initiative segment was short and thin than middle segment, the angle between uterine artery and it's mother artery was variable. 45° oblique angiography can demonstrate the uterine arterial panorama. Conclusion 45° oblique angiography was helpful for displaying the detailed distributing of uterine artery. 4.0-5.0F catheter could complete the superselective angiography of uterine artery; the successful factor of catheterization was to recognize the angle of initiative segment of uterine artery. |
