| 目的 分析对比二维超声,彩色多普勒血流显像(CDFI)及CA125的准确性,为卵巢良恶性肿瘤鉴别诊断寻求最佳方法。方法 术前应用二维超声及CDFI对72例附件肿块进行形态结构观察及血流分析,测定最小RI值,同时抽取外周血测定CA125,并与术后组织学对比。结果 ①二维超声可以对具有典型征象的良性肿瘤作出正确诊断;②卵巢良恶性肿瘤之间RI及CA125值有显著性差异(P<0.01);③以上述三种方法逐一筛选检出良性病变,敏感性96.15%,特异性95.65%。结论 二维超声辅以CDFI及CA125测定,以逻辑排除法鉴别卵巢良恶性肿瘤能提高诊断准确性。 |

| Objective To assess the diagnostic accuracies of sonography,color Doppler flow imaging(CDFI),and serum CA125,and to provide the best methed for distinguishing between benign and malignant ovarian masses. Methods Seventy-two cases of ovarian masses were examined.The characteristics of ultrasound morphology,minimum resistance index (RImin)detected by CDFI,as well as serum level of CA125 measured by solid-phase immunoradimetricassary were analyed and compared with histopathologic findings. Results ①The benign ovarian lesions which had typical sonographic features might be diagnosed correctly.②Between benign and malignant ovarian lesions,the RImin and the serum levels of CA125 had significant difference (P<0.01).③To detect benign lesions from all ovarian masses based on the above three diagnositic modalities,the logical exclusiveness methods had sensitivity 96.15%,specifity 95.65%. Conclusion By combinating sonography with CDFI and serum CA125,the logical exclusiveness methods were accurate in distinguishing between benign and malignant ovarian lesions. |
