      目的 探讨高血压病患者诊所或动态脉压与左心功能之间的相互关系。方法 选择555名初诊或停药2周以上的轻中度高血压病患者进行诊室血压、24小时动态血压监测、超声心动图检查。555名患者根据诊室脉压(CPP)及24小时动态脉压(APP)水平分别分为四组,A组:PP≤40mmHg;B组:40mmHg60mmHg。结果 随24h动态脉压增大,左室舒张功能减退,主要表现在心房收缩期与舒张早期充盈峰值流速之比值(A/E)增加,舒张早期充盈时间(EDFP)延长。左室收缩功能无明显变化。24h动态脉压较诊室脉压与左室舒张功能的相关性更明显。结论 脉压增高致高血压病患者左室舒张功能减退,而对左室收缩功能无明显影响。24h动态脉压较诊室脉压与左室舒张功能的相关性更强。

      Objective To investigate the relationship between pulse pressure (PP) and left ventricular function in patients with essential hypertension. Methods Clinical blood pressure, 24h ambulatory blood pressure monitoring, two-dimensional echocardiography were examined in 555 initially untreated patients. The patients were divided into four groups according to clinical pulse pressure (CPP) or ambulatory pulse pressure (APP) respectively: Group A: PP≤40mmHg; Group B: 40mmHg<PP≤50mmHg; Group C: 50mmHg<PP≤60mmHg; Group D: PP>60mmHg. Results With increased clinical or ambulatory pulse pressure, left ventricular diastolic function decreased. It was manifested by increased ratio of peak flow velocity of atrial contraction to early diastole (A/E) and prolonged early diastolic filling period (EDFP). There were no changes in systolic function with increased pulse pressure. The correlation between ambulatory pulse pressure and diastolic function was stronger than that between clinical pulse pressure and diastolic function. Conclusion Increased pulse pressure induces decreased left ventricular function in patients with essential hypertension. Ambulatory pulse pressure is a better way than clinical pulse pressure in evaluating left ventricular diastolic function.
