      目的 探讨三维彩色血管能量成像对肾肿瘤的诊断价值。方法 应用3D-CPA检测不同病理类型肾肿瘤46例,评定血管分型(I~III型),计算肿瘤血管指数(VI)。结果 ①36例肾恶性肿瘤中3D-CPA III型26例,II型10例;10例良性肿瘤3D-CPA I型6例,II型4例。以3D-CPA III型诊断肾恶性肿瘤敏感性为72.2%,特异性为100%。②恶性肾肿瘤VI 为(0.3418±0.1556)条/cm3,良性肾肿瘤VI为(0.1948±0.0573)条/cm3,二者比较差异有显著性意义(P<0.05)。以VI>0.25条/cm3诊断恶性肾肿瘤敏感性77.8%,特异性80%。结论 3D-CPA可清晰、完整、立体显示肾肿瘤的血管分布,其血管分型、血管指数有助于肾肿瘤的诊断和鉴别诊断。

      Objective To evaluate the diagnostic value of renal tumor with 3D-CPA. Methods Forty-six patients with different pathological types of renal tumors were examined with 3D-CPA.The vascular pattern of renal tumor and vascularity index(VI) were assessed. Results Twenty-six of the 36 renal malignant tumors were 3D-CPA III,10 of which were 3D-CPA II.Six of 10 renal benign tumors appeared 3D-CPA I,4 of which appeared 3D-CPAII.Using the vascular pattern of 3D-CPA III,the sensitivity and specificity in diagnosing renal cell carcinoma were 72.2% and 100% respectively.VI was (0.3418±0.1556) counts/cm3 in kidney malignant tumors and was (0.1948±0.0573) counts/cm3 in kidney benign tumors.There was significant difference between VI of malignant and benign tumors.Using the VI more than 0.25 counts/cm3 as a standard,the sensitivity and specificity in diagnosing kidney malignant tumor were 77.8% and 80% respectively. Conclusion 3D-CPA could show overall vascular perfusion of different renal tumors clearly.Vascular pattern and VI of 3D-CPA are helpful to diagnose and differentiate renal tumors.
