      目的 通过对糖负荷、化疗及供应肿瘤的血流量等三种影响肿瘤氟脱氧葡萄糖(18F-FDG)摄取因素的实验研究,为临床上行18F-FDG SPECT/PET符合显像时合理控制影响因素及正确解释显像结果提供理论依据。方法 荷瘤(H22肝癌)昆明种小鼠30只,随机分为糖负荷组、化疗组及血流组,前两组又各分为对照组与实验组,实验组分别用50%的葡萄糖(质量分数)0.5ml灌胃及静脉化疗,血流组同时用99Tcm标记的人血白蛋白(99Tcm-HSA)测定肿瘤血流量,各组均静脉注射18F-FDG 185kBq/0.2ml,分别观察糖负荷与化疗对肿瘤摄取18F-FDG的影响,并分析肿瘤18F-FDG摄取与肿瘤血流量之间的相关性。结果 ①糖负荷组血糖浓度较对照组显著升高(6.59±0.86 vs 8.87±1.56mmol/L,t=2.56,P<0.05),其标准摄取值(standard uptake value,SUV)较对照组显著降低(2.694±0.415 vs 1.904±0.338,t=3.10,P<0.05)。②化疗组血糖浓度与对照组无显著性差异(5.77±1.81 vs 5.31±1.79mmol/L, t=0.40,P>0.05), 其SUV值较对照组显著降低(1.22±0.27 vs 0.87±0.14, t=2.58,P<0.05)。③血流组18F-FDG与99Tcm-HSA摄取之间存在正相关(r=0.7731)。结论 血糖浓度升高、化疗可使肿瘤18F-FDG摄取减少,肿瘤对18F-FDG的摄取值与供应肿瘤的血流量之间存在正相关。

      Objective To investigate three kinds of factors: glucose load, chemotherapy and blood flow volume,which influencing the uptake of 18F-deoxyglucose (FDG) in tumor to provide theoretical basis for clinical control of these factors and make correct diagnosis in FDG imaging. Methods Thirty mice with H22 hepatic carcinoma were randomly divided into three groups: glucose load group, chemotherapy group and blood flow metering group. The first two groups were subdivided into control group and experimental group, the experimental group was given 50% glucose 0.5ml orally or intravenous chemotherapy, respectively. All the mice were intravenously injected 18F-FDG 185kBq/0.2ml, while the third group was given 99Tcm-human serum album (99Tcm-HSA) 185kBq/0.2ml intravenously 50 minutes later. Sixty minutes after FDG injection, the mice were killed and tumors were excised. Radioactivities of 18F-FDG and 99Tcm-HSA were determined with a well-type gamma counter. Influence of glucose load and chemotherapy on the uptake of FDG and the correlation between the uptake of FDG and tumor blood flow volume were analyzed, respectively. Results ①In glucose load group, the blood glucose level was significantly higher (6.59±0.86 vs 8.87±1.56mmol/L, t=2.56, P<0.05) while the standard uptake value (SUV) in the tumor was significantly lower (2.694±0.415 vs 1.904±0.338, t=3.10, P<0.05) in experimental group than in the control group. ②There was no significant difference of blood glucose level (5.77±1.81 vs 5.31±1.79mmol/L, t=0.40, P>0.05) between the experimental group and the control group, while the SUV in the tumor was significantly lower (1.22±0.27 vs 0.87±0.14, t=2.58, P<0.05) in the experimental group than in the control group. ③There was a positive correlation (r=0.7731) between the SUV of 18F-FDG and 99Tcm-HSA in blood flow metering group. Conclusion Elevated blood glucose level and chemotherapy can diminish the uptake of FDG in tumor; there is positive correlation between the uptake of 18F-FDG and the blood flow volume in tumor.
