目的 研究超声监测大鼠失血性休克过程肝脏血流动力学改变的可行性及可靠性。 方法 在大鼠失血性休克前、后及再灌注24h内,用7.5MHz探头对大鼠肝脏进行二维超声(2DUS)、彩色血流速度图(CVI),脉冲多普勒(PW)检查,测量PV、肝动脉(HA)血流参数。 结果 休克期肝血流动力学改变早于肝生化指标的改变,表现为肝内血流分布减少;PV流速降低;HA的 Vmin降低,RI升高(P<0.01)。再灌注期肝内血流分布增多;HA的Vmax、Vmin较休克期升高,RI下降,有非常显著性差异(P<0.01);但RI值与休克前比较仍有显著性差异(P<0.05),且PV流速与休克期比较,无显著性差异(P=0.29)。 结论 无创性超声检测血流可作为监测失血休克大鼠模型肝脏血流动力学改变的可靠方法。 |

Objective To study the feasibility and reliability of hemodynamic changes of liver in hemorrhagic shock rat by ultrasonography. Methods Ultrasonography had been successfully introduced to the detection of hepatic hemodynamic changes in hemorrhagic shock rat. 2DUS,color velocity imaging (CVI),and pulsed wave Doppler (PW) of liver were performed before (n=20),30-90 minutes after hemorrhagic shock (n=18) and within 24 hours after resuscitation (n=7). The peak-systole velocity (Vmax),end-diastolic velocity (Vmin),resistance index (RI) of hepatic artery (HA) and velocity of portal vein (PV) were measured.Results After hemorrhagic shock,blood distribution of liver in hemorrhagic shock rat was decreased; the velocity of PV and Vmin of HA were significantly decreased but RI of hepatic artery were significantly increased (P<0.01). After resuscitation,hepatic blood distribution and velocity of HA were increased obviously but RI was lower significantly than that at the stage of the chock (P<0.01); although RI of HA rose,there was remarkable difference compared with that before the chock (P<0.05),and the velocity of PV had no significant changes (P=0.29). Conclusion Ultrasonography can be used to detect hemorrhagic shock rat and assess the hemodynamic changes of the liver accurately and non-invasively. |
