      目的 研究心肌背向散射积分(IBS)及IBS周期变化幅度(CVIB)用于检测基因转染猪慢性缺血心肌治疗的价值。 方法 在猪冠脉左旋支(LCX)放置Ameroid环建立慢性心肌缺血模型,4周后分组注射携带血管内皮生长因子-B基因的重组腺病毒液(Ad.VEGF-B)或磷酸盐缓冲液(PB)。应用背向散射积分技术联机分析技术,观测正常状态、置环后2周、4周及注射Ad.VEGF-B液或PB液2208062后2周、4周左室乳头肌短轴观侧后壁标化平均背向散射积分(%AII)和背向散射积分周期变化幅度(CVIB)的变化。 结果 对照组置环后2周、4周及PB对照注射后%AII逐渐升高,而CVIB逐渐降低;基因转染组置环后2周、4周%AII和CVIB变化同对照组,注射后2周和4周%AII较对照组明显降低,CVIB较对照组明显增加。 结论 IBS能早期、快速、准确地检测慢性缺血心肌基因转染治疗前后心肌结构和室壁运动改善的情况,为临床研究和治疗冠心病提供可靠指标。

      Objective To investigate the value of ultrasound integrated backscatter (IBS) and cyclic variation of integrated backscatter (CVIB) in detecting chronic ischemic myocardium before and after gene transfer. Methods An Ameroid constrictor was placed around the proximal left circumflex artery (LCX) of each open-chest pig. Four weeks later,groupⅠwas directly intramyocardially administrated replication-deficient recombinant adenovirus coding for the vascular endothelial growth factors B (Ad. VEGF-B),and groupⅡ was administrated phosphate buffer (PB). The calibrated average image intensity (%AII) and cyclic variation of integrated backscatter (CVIB) in lateral-posterior wall of left ventricular papillary muscle level short axis view (LVPM-SAM) of each period (normal state,2 weeks and 4 weeks after Ameroid constrictors placement,2 weeks and 4 weeks after intramyocardially administration) were measured. Results %AII of control group increased gradually whereas CVIB decreased after Ameroid constrictors placement and PB administration. The changes of %AII and CVIB in groupⅠbefore gene transfer was the same as in control group. After vector administration,%AII decreased and CVIB increased significantly compared with those of control group. Conclusion Ultrasonic integrated backscatter tissue characterization can early,rapidly and accurately detect myocardium structure and wall motion changes in every stage of chronic ischemic myocardium before and after gene transfer,and provide more information and more help for clinical investigation and treatment of CHD.
