| 目的 探讨子宫动脉栓塞术的方法学,提高手术成功率。方法 对103例经临床、妇检及影像学明确的子宫肌瘤患者行子宫动脉栓塞术,并总结手术操作过程中成功与失败的经验。结果 103例患者207条子宫动脉(1例右侧为双子宫动脉)插管成功206条,插管成功率99.5%,单侧穿刺完成双侧子宫动脉插管的101例,占98.1%,2例行双侧穿刺插管,占1.9%,栓塞子宫动脉204条,栓塞成功率98.6%,3例行单侧栓塞。操作过程中出现子宫动脉痉挛82条,占39.6%,其中2条完全闭塞,2条出现夹层,未发现与返流有关的并发症。结论 熟悉盆腔动脉的解剖特点、规范的操作、选择适宜导管及栓塞物的合理应用对手术成功至关重要。 |

| Objective To evaluate the techniques and methods of uterine artery embolization (UAE) in order to improve the treatment of uterine myomas. Methods One hundred and three women who were confirmed uterine myomas clinically and radiographically were performed UAE.The experiences in this procedure were concluded and analyzed. Results Among 207 uterine arteries in the 103 women (one women with double right uterine arteries),206 of them were cathetered successfully (99.5%).And 101 cases were performed by unilateral puncture to embolize bilateral uterine arteries (98.1%).Two cases were treated with bilateral femoral access (1.9%).Among the 207 arteries,there were 204 arteries embolized successfully (98.6%).During the operation,vessel spasm occurred in 82 uterine arteries (39.6%),occlusion and dissection were observed in 2 arteries respectively,other complications related to regurgitation didn't happen. Conclusion The successful operation depends on skillful operating,selection of suitable catheter and particles,in addition to being fimiliar with the anatomy of pelvic arteries. |
