| 目的 探讨成人股骨头缺血坏死的MR特征性表现及其诊断价值。方法 前瞻性研究100例疑诊成人股骨头缺血坏死病人,从中挑选出经随访证实的70例股骨头缺血坏死病人髋关节的影像学资料进行分析和总结。结果 MR表现:121个坏死股骨头中,111个以前上部为著,4个位于外上方,3个累及整个股骨头,3个多灶性分布;39个股骨头病灶中心T1WI高信号,T2WI中等信号;82个T1WI及T2WI均呈高低混杂信号;118个股骨头出现"线样征",其中包括单线32个、双线51个、三线16个、四线19个;3个股骨头无"线样征";64个股骨头出现"骨髓水肿";28个股骨头"线样征"内或周边出现(类)圆形影。结论 "线样征"有助于早期诊断本病。MR诊断股骨头缺血坏死的敏感性明显优于X线平片和CT,与ECT相当。 |

| Objective To study the characteristics of the femoral head necrosis (FHN) on MRI. Methods The MRI findings of 70 cases with FHN were analyzed. Results Among 121 hips of FHN,111 lesions were found in anterior-upper,4 in posterior-upper,3 in the whole femoral head and 3 hips in different parts of femoral head.Thirty-nine hips revealed high intensity on T1WI and intermediate signal intensity on T2WI,and 82 were with low to high signal intensity on both T1WI and T2WI.There were 118 hips with line sign,including 32 femoral heads with single-line,51 with double-line,16 with three-line and 19 with four-line.There were 3 hips without line sign.Sixty-four femoral heads had marrow edema and 28 femoral heads had the cystic regions. Conclusion Line sign is helpful to diagnose FHN early.MRI is superior to CT or conventional radiography in imaging diagnosis and equal to ECT. |
