目的 探讨18F-FDG对肺癌的诊断价值。方法 28例肺部占位性病变行18F-FDG符合线路SPECT显像。经过全能量X线衰减校正和有序子集最大期望值法(COSEM)叠代重建后,将CT图像与核医学图像(PET)进行同机图像融合,获得横断面、矢状面、冠状面三维断层图像。计算病灶与对侧相应部位的摄取比值R(T/N)。分别采用视觉分析和半定量分析比较18F-FDG对肺癌的诊断效能。结果 以摄取比值R≥1.7为判断标准,诊断肺癌的灵敏度为100%,特异性88.2%,准确性92.9%。而单纯视觉分析对肺癌的诊断灵敏度为100%,特异性为76.5%,准确性85.7%。11例肺癌患者18F-FDG显像均为阳性,ROI分析最大摄取比值达13.37,最小为2.36。本组有2例肺结核病例18F-FDG显像呈假阳性,摄取比值为2.8及4.9。结论 18F-FDG 符合线路显像对肺癌诊断具有高灵敏度、高特异性和高准确性,不仅肺癌病灶部位能清晰显示,而且对纵隔淋巴结、锁骨上淋巴结、腋窝淋巴结转移的诊断具有重要的意义,为临床制定治疗方案、观察疗效和疾病分期提供重要的临床信息。 |

Purpose To investigate the diagnostic value of 18F-FDG coincidence SPECT on lung cancer. Methods 18F-FDG coincidence SPECT imaging was performed on 28 patients with space-occupying lesions in the lungs.Upon whole energy X-ray attenuation correction and cosem reconstruction,we add CT to PET co-registration in the same unit,acquiring axial,sagittal and coronal three-dimensional images.Uptake ratio (R) of lesion to contralateral region was calculated.Visual analysis and semi-quantitative method were used to evaluate the diagnostic efficacy of 18F-FDG upon lung cancer. Results If uptake ratio ≥1.7 as a threshold,the diagnostic sensitivity,specificity and accuracy for lung cancer were 100%,88.2% and 92.9% respectively;whereas for visual analysis alone,the sensitivity was 100%,specificity 76.5% and accuracy 85.7%.Eleven patients of lung cancer were all positive for 18F-FDG imaging.The uptake ratio ranged from 2.36 to 13.37.Two false positives occurred.They were pulmonary tuberculosis patients,with uptake ratios 2.8 and 4.9. Conclusion 18F-FDG coincidence imaging was highly sensitive,specific and accurate for diagnosing lung cancer.Apart from clearly localizing the lesion,it also revealed mediastinal supracla ricular,axillary nodal metastasis.Thus it provides important clinical information for treatment planning,clinical staging,therapeutic effect observation. |
