| 目的 研究眼眶炎性假瘤的MRI表现特征与类型,探讨眶内炎性假瘤的诊断与鉴别诊断。方法 21例患者为手术病理或随访结果证实,均行MRI普通扫描与脂肪抑制下增强扫描。结果 依据肿瘤发生部位可分为4型:①眼肌型,炎性假瘤主要累及眼外肌与眶壁,使眼外肌肥厚,眶壁骨质受压变形变薄;②肿块型,炎性假瘤呈均匀信号椭圆形肿块,位于肌锥内,常包绕视神经,但视神经清晰可见;③弥漫型,炎性肿块呈广泛充填球后部分,形成"眼眶铸型"外观,并使患侧眼眶增大,但视神经仍可见;④眶尖型,炎性肿块呈锥形充填眶尖,并使其扩大,部分肿块呈短T2等T1信号。增强压脂扫描:所有炎性肿块均明显均匀性强化。结论 MRI能够准确而清晰地显示眼眶炎性假瘤的信号特征、发生部位、与视神经、眼外肌的关系,结合病史,能够做出定位、定性诊断。 |

| Objective To investigate the MRI findings of orbital inflammatory pseudotumor for its diagnosis and differential diagnosis. Methods Twenty-one cases of orbital inflammatory pseudotumor were confirmed by pathology or follow-up survey.MRI pre-contrast and post-contrast with fat suppression were performed in all cases. Results There were four types according to the position of tumor:①Eye muscle type:The pseudotumor encroach the muscle and orbit of eye,with hypertrophy of the muscle.The bones of orbit were pressed to attenuation.②Tumor type:Pseudotumor looked like oval bump with uniformity signal,laying in muscle-wimble,and it circled optic nerve,but the optic nerve was clear.③Permeating type:Inflammatory mass was full in the retrobulbar place,and manifested "casting orbit" sign.④Tine of orbit type:Inflammatory mass was full in the tine of orbit,and enlarged it.Parts of masses had short T2 and equal T1 signals.All of pseudotumors were symmetrically enhanced after fat suppression enhancement. Conclusion MRI can display the signal character,location,relation between optic nerve and enhancement of pseudotumor exactly and clearly. |
