目的 探讨18F-FDG PET对舌癌诊断、术前分期及颈部淋巴结转移判定的应用价值。方法 10例病理确诊舌癌,PET显像与手术后病理结果对比。结果 舌癌病灶处均可见葡萄糖代谢异常增高,SUVmax=5.49±3.71,较正常舌(SUVmax=1.13±0.33)代谢增高明显。6/7例PET结果与术后病理一致。4例PET显像未见颈部转移灶的患者均得到术后病理证实,惟一1例颈部转移淋巴结病理阳性而PET阴性的病例可能为化疗抑制了病灶组织代谢活性所致。结论 PET 可准确显示舌原发病灶范围,据此科学确定舌部病灶切除的安全缘。此外,PET有望作为舌癌术前是否需行颈部清扫术的指征,以改变目前无论分期如何,一律行颈清扫的现状。进一步确认尚待大样本、多中心的研究证实。 |

Objective To investigate the value of PET in staging of tongue neoplasms. Methods All PET image were compared with histopathology. Results Hypermetabolism could be seen in all the original foci (SUVmax=5.49±3.71 in original focus vs 1.13±0.33 in countralateral healthy tissue).PET were consistent with pathology in 6/7 cases.The only false one might be caused by chemotherapy before PET scan. Conclusion PET could show the scope of the lesion accurately,and be served as the "gate-keeper" of neck obliteration in tongue neoplasms.Big sample and multi-center investigation were needed to further identify this viewpoint. |
