      目的 探索脑梗死在超急性期平扫CT表现规律,提高其诊断价值。方法 总结了发病6h之内脑梗死123例患者的一般资料和临床表现,分析了平扫CT表现,将其分为灰质组(69例)和白质组(18例)和基底节组(36例)。结果 平扫CT阴性表现51例,占41.5%,其中灰质组的阴性率显著低于另两组(P<0.001)。阳性表现主要有脑动脉高密度征(25例)、局部脑肿胀征(58例)和脑实质低密度征(22例)。其中灰质组的局部脑肿胀征出现率显著高于白质组和基底节组(P<0.001)。其他表现有陈旧性脑梗死20例、老年脑改变35例。结论 仔细分析平扫CT影像,对半数以上的超急性期脑梗死病例可作出诊断或提示性诊断。对病变主要发生在脑表面灰质区的病例,CT平扫有更大的诊断价值。

      Purpose To explore the regular pattern of plain CT scan in superacute cerebral infarction patients. Methods The general material and clinical manifestations of 123 cases with cerebral infarction within 6h were summarized.The findings in plain CT scan were analysed.They were divided into three groups:Gray matter group (69 cases),white matter group (18 cases) and basal ganglion group (36 cases). Results CT plain scan were negative in 51 cases (41.5%).The negative rate in gray matter group was significantly lower than that in white matter group and basal ganglion group (P<0.001).The positive signs included mainly the hyperdense cerebral artery sign (25 cases),the focal cerebral swelling sign (58 cases) and the hypodense cerebral parenchyma sign (22 cases).The incidence of the focal cerebral swelling sign in gray matter group was significantly higher than that in white matter group and basal ganglion group (P<0.001).Other findings included the chronic cerebral infarction (20 cases) and the senile brain findings (35 cases). Conclusion When CT plain scan images are carefully analysed,the diagnosis or suggestive diagnosis of superacute cerebral infarction may be worked out in more than half cases.The value of CT plain scan is greater for the cases with the lesion mainly in gray matter area of cerebral surface.
