      目的 评价三维磁共振波谱成像(three-dimensional MR spectroscopic imaging,3D MRSI)检查前列腺的可行性并初步评价在MRI基础上联合应用3D MRSI对鉴别前列腺癌和正常前列腺外周带与良性前列腺增生的价值。方法22例疑为前列腺癌的患者行MRI和3D MRSI检查。将3D MRSI的检查结果与病理结果对照。结果22例患者中有21例完成了前列腺3D MRSI检查。1例患者因既往曾行直肠肛门手术而无法置入直肠内线圈致使检查不能完成。6例患者获得41处病理结果,其中21处为前列腺癌,14处为前列腺增生,6处为正常外周带。计算3D MRSI中相应位置的[胆碱(choline)+肌酸(creatine)]/枸椽酸盐(citrate)的比值,发现前列腺增生和正常前列腺外周带的 (choline+creatine)/citrate值较小(0.51±0.17和0.60±0.21),而前列腺癌的(choline+creatine)/citrate值较大(1.98±0.95)。结论3D MRSI检查对于多数前列腺疾病患者是可行的,可得到有诊断意义的波谱信息。与病理对照研究的初步结果显示3D MRSI有助于鉴别肿瘤与其他疾病,与MRI结合能直观地显示肿瘤的范围。

      Objective To evaluate the feasibility of in vivo three-dimensional MR spectroscopic imaging (3D MRSI) and primarily evaluate the value of 3D MRSI. Methods Combined phased-array, endorectal MR imaging and 3D MRSI was performed in 22 patients suspected of prostatic adenocarcinoma. MR imaging and 3D MRSI data were compared with histo-pathological findings. Results Twenty-one of the 22 patients accomplished the examination and obtained the acceptable 3D MRSI results. One patient could not endure the endorectal coil because of a previous operation of his annus rectum. Forty-one pathological samples were obtained from 6 patients, which were comparable with 3D MRSI results. Two-one samples of them were prostate adenocarcinoma, whereas 14 were benign prostate hyperplasia and 6 were normal peripheral zone. Significant variations in metabolite levels with normal peripheral zone, BPH and prostate cancer were detected with a 3D MRSI examination added to a clinical MR imaging examination. The ratio of (choline+creatine)/citrate of the normal peripheral zone, BPH and prostate cancer was 0.51±0.17, 0.60±0.21 and 1.98±0.95 respectively. An estimate of prostate cancer was determined by generating metabolite images in which the metabolite ratio significantly exceed normal peripheral zone values. Conclusion 3D MRSI is feasible in the examination of the prostate gland. The preliminary results suggest that a 3D MRSI added to a clinical MRI examination may help define the presence and spatial extent of prostate cancer.
