| 目的
评价CT扫描对胸段食管癌纵隔淋巴结转
移的诊断标准。 方法 93例CT资料完整经手术治疗胸中下段食
管癌患者,统计CT纵隔淋巴结肿大情况与术后病理对照。结果 以3
mm、5mm、10mm、15mm短径为标准,纵隔淋巴结转移PPV分别为29.8%,4
4.4%,62.5%,75%。以纵隔淋巴结短径大于5mm为标准,C
T对纵隔淋巴结肿大的敏感性、特异性、准确性分别为82.35%,85.5%,84.9%
。对右上气管旁沟组、主动脉窗组PPV>50%。
结论 CT扫描有利于发现纵隔异常肿大的淋巴结,对单个
淋巴结短径大于5mm、或一区域多个小结节均应作为临床治疗标准。 |

| Objective To evaluate the diagnostic criterion for mediastinal lymph node metastasis of esophageal carcinoma by CT. Methods CT scans were performed in 93 patients with esophageal carcinoma who proved by operation. Pathologic findings were compared with the CT findings. Results If the criterions for lymph node metastasis on CT scans were 3mm, 5mm, 10mm, 15mm in short-transverse diameter, the positive predictive value (PPV) were 29.8%,44.4%,62.5%,75% respectively. The criterion was≥5mm on CT scan in short-transverse diameter, the sensitivity was 82.35%, specificity 85.5%, and accuracy 84.9% respectively. Conclusion CT provided a helpful assessment of mediastinal lymph nodes metastasis in patients with esophageal carcinoma for clinic. The criterion for lymph node metastasis on CT scans was ≥5mm in the short diameter. |
