| 目的
使用超声心动图评价二叶主动脉瓣畸形(BAV)与升主动
脉扩张的关系。方法 使用二维超声在四个部位检测42例主动脉瓣功能正
常的BAV患者(病例组1)、37例主动脉瓣狭窄的BAV患者(病例组 2)和40例
年龄、体表面积、血压等匹配的健康人(对照组)的升主动脉内径:瓣环
、瓦氏窦、主动脉上嵴及升主动脉近端。结果 病例组2升主动脉瓦氏窦、
主动脉上嵴及近端
内径最大,其次为病例组1 (P<0.05~P<0.001)。但病例组1中有一部分BAV患者的
升
主动脉内径与对照组无显著差异。结论 BAV常合并升主动脉扩张,提示两
者可能为同一发育异常,即先天性主动脉壁发育缺陷的不同表现形式。 |

| Objective
To determine whether the ascending aortic dilation ass
ociated with a bicuspid aortic valve occurs independently of valvular hemodynami
c abnormality. Methods Aortic dimensions were measured by two-ec
hocardiography i
n 79 subjects with a functionally normal (n=42) or stenotic (n=37) bicus
pid aort
ic valve and compared with the findings in 40 normal subjects matched for age, body
surface area and artery pressure. Ascending aortic measurements were made at fo
ur levels: annulus, sinuses of Valsalva, supra-aortic ridge, and proximal ascend
ing aorta. Results The dimensions of the ascending aorta in su
bjects wit
h a bicuspid aortic valve were larger than in controls at three levels:sinuses o
f Valsalva, supra-aortic ridge, and proximal ascending aorta (P<0.05 to P
<0.001)
. [BP(]There were 18 subjects had ascending aortic dimensions comparable with control
s, while 24 had definitely abnormal ascending aortic dimensions in 42 subjects w
ith a functionally normal bicuspid aortic valve.[BP)] Conclusion Ther
e was a high pre
valence of ascending aortic dilatation in patients with bicuspid aortic valve. Thes
e findings support the hypothesis that bicuspid aortic valve and ascending aorti
c dilation may reflect a common developmental defect. |
