      目的 评价三维脂肪抑制快速毁损梯度回波序列(3D FS-FSPGR)对髌股关节软骨病变的诊断能力。方法 5例尸体膝关节,对1例髌股关节用不同参数的3D FS-FSPGR序列轴位扫描,找出软骨/液体和软骨/骨髓对比噪声比最佳的参数。用该参数的3D FS-FSPGR序列扫描另外4例髌股关节。所有髌股关节均另外作3D FS-SPGR序列扫描。用双盲法对髌股关节的大体标本和MRI进行软骨病变的评价,计算每种序列诊断髌股关节软骨病变的敏感性、特异性和总符合率。结果 3D FS-FSPGR序列扫描时间为1分32秒,其诊断髌股关节软骨病变的敏感性、特异性和总符合率为92%、71%和85%;3D FS-SPGR序列扫描时间为4分55秒,其诊断髌股关节软骨病变的敏感性、特异性和总符合率为92%、86%和90%。结论 3D FS-FSPGR序列对软骨病变的诊断能力与3D FS-SPGR相当,但扫描时间仅约为其1/3。

      Objective To evaluate the role of 3D FS-FSPGR sequence in the assessment of abnormalities of patellofemoral joint cartilage. Methods Five cadaveric knees were injected 40ml of saline prior to imaging. Optimization of the 3D FS-FSPGR sequence was achieved by axial imaging the first cadaveric knee. The other 4 cadaveric knees were imaged by the sequence which had the highest contrast-to-noise ratio for cartilage/fluid and cartilage/bone marrow. Three-dimensional fat-suppressed spoiled gradient recalled echo (3D FS-SPGR) sequence also was used in imaging of all 5 cavaveric knees. The macroscopic sections and MR images of each region were evaluated by two orthopaetic doctors and two radiologists respectively. The sensitivity,specificity and accuracy of each sequence were calculated. Results The acquisition time of 3D FS-FSPGR sequence was 1 minute 32 seconds,the sensitivity,specificity and accuracy of which were 92%,71%,85%. The acquisition time of 3D FS-SPGR sequence was 4 minutes 55 seconds,the sensitivity,specificity and accuracy of which were 92%,86%,90%. Conclusion 3D FS-FSPGR sequence had similar role to 3D FS-SPGR sequence in the assessment of abnormalities of patellofemoral joint cartilage and took about one-third acquisition time that 3D FS-SPGR sequence did.
