      目的 评价老年2型糖尿病患者下肢动脉彩超的诊断价值。方法 彩超检测老年2型糖尿病患者下肢动脉内径、中-内膜层(IMT)、粥样斑块及血流频谱,与同龄正常人进行比较。结果 ①患者组IMT、粥样斑块较对照组明显增厚(P<0.01);②患者组下肢动脉明显狭窄段、闭塞段检出率明显高于对照组(14.89%和3.10%、1.06%和0.27%,P<0.01),89.29%闭塞段位于膝关节以下动脉;③有下肢症状组动脉狭窄和闭塞检出率较无症状组高;④病程与明显狭窄和闭塞检出率无明确关系。结论 彩超可清楚地显示老年2型糖尿病患者下肢动脉IMT和斑块,结合彩色多普勒血流显像(CDFI)及频谱检测可以判断狭窄程度或闭塞。

      Objective To investigate the diagnostic value of low extremity artery colour Doppler imaging in elderly patients with type 2 diabetes mellitus (DM). Methods The elderly patients with DM accepted the examination of low extremity artery colour Doppler imaging.The inner caliber,IMT, atheromatous plaques and blood flow spectrum of lower extremity artery were measured and compared with the control group's. Results ①the thickness of IMT and atheromatous plaques were thicker than the control group's (P<0.01);②the check-out rates of marked narrow segments and occlusive segments in the low extremity artery in patients' group were significantly higher than the control group's (14.89% and 3.10%,1.06% and 0.27%,P<0.01,respectively);③the check-out rates of marked narrow segments and occlusive segments in patients with low extremity artery symptoms were higher than patients without symptoms;④ the check-out rates of marked narrow segments and occlusive segments were not related with the duration of the disease. Conclusion The narrow extent and occlusive segments could be judged by united with CDFI and the measurement of frequency spectrum, and the measurment is especially suitable for the small arteries under knees.
