目的 通过分析嗅神经母细胞瘤的影像学表现,探讨其在临床诊断和分期中的价值。 方法 经病理证实的嗅神经母细胞瘤共18例,将其影像学资料进行回顾性分析,包括肿瘤中心位置、侵及范围、骨破坏、有无钙化、出血和积液以及信号和密度的改变等,并与手术结果进行对照。 结果 ①肿瘤中心:多位于上鼻腔和筛窦;②对于显示肿瘤侵犯的范围,MRI较CT更敏感;③肿瘤分期: A期0例,B期2例,C期16例。伴颈部淋巴结转移6例,其中1例伴颈椎及一侧睾丸转移;④信号和密度的改变:T1加权像肿瘤信号均匀,与肌肉信号相近,但低于脑灰质信号,T2加权像肿瘤表现为稍高或等于脑灰质信号,注射造影剂后呈中等或明显强化,多数强化不均匀。CT表现为中等较均匀密度,轻至中度强化。 结论 MRI和CT可确定肿瘤大小、侵犯范围以及与周围血管或神经的关系, MRI可以多方位、多参数扫描,在监测治疗效果、评估预后等方面优于CT,同时可以早期发现肿瘤复发,为临床治疗方案的制定提供必要的参考依据。 |

Objective To evaluate the value of MRI in the diagnosis and staging of esthesioneuroblastoma. Methods We reviewed 18 cases of histologically proven ganglioneuroma, the contents of studies were included the origination, extent of tumor, bone destruction and calcification, hemorrhage, obstructive disease, signal or density changes, and etc. The results were compared with surgical findings. Results ①Center of tumor:superior nasal cavity and ethmoid sinus were the most common sites; ②MRI was superior to CT in evaluate extent of tumor; ③Staging: stage A: 0 case, stage B: 2 cases, and stage C: 16 cases, accompanied with cervical LNM 6 cases, and 1 of the 6 cases also companied with cervical spine and testicle metastasis;④Signal and density changes:as compared to gray matter, the signal on T1WI was hypointense, and on T2WI was either iso-or hyperintense slightly, contrast enhancement was either moderate or obvious, and of variable uniformity,the tissue density on pre-contrast CT was equal to that of muscle and was relatively homogenous. Following administration of intravenous contrast, mild and moderate enhancement was found.Conclusion MRI and CT are helpful for evaluating the size, extent of tumor, and its relationship to vessels or nerves. Because of its multiple direction and parameters, MRI was superior to CT on supervising the effect of treatment, estimating the future of tumor,and meanwhile, it can find the recurrence of tumor earlier, so it provide more useful information for clinical planning. |
