      目的 探讨经皮穿刺微波凝固疗法在正常动物肝脏的凝固效果。方法 在超声引导下对7头家猪进行了经皮肝脏穿刺微波凝固疗法。共取得24个凝固区。凝固温度设定在65℃,凝固时间为5min,输出功率为40W。一周后处死动物,取出肝脏测量凝固坏死区的大小,然后取标本进行病理检查。行微波凝固疗法前、后及一周后分别取血进行肝功能检查。结果 肝脏凝固坏死区肉眼呈白-淡粉色,形状为椭圆形,其长、短轴长度分别为(30.08±5.83)mm及(14.75±3.66)mm。病理学检查可见肝细胞水肿,变性及凝固性坏死,还可见炎性反应及纤维组织增生,以及微脓肿形成,出血及巨细胞反应。治疗前后的肝功能无显著变化。结论 微波凝固疗法能够有效引起肝脏组织的凝固坏死,为治疗肝脏肿瘤提供了一种新的方法。

      Purpose To evaluate the efficacy of percutaneous microwave coagulation therapy (PMCT) in normal swine liver. Methods Seven swine were undergone percutaneous microwave coagulation under ultrasound guidance. A total of 24 sites of puncture were obtained. The temperature was set at 65℃, whereas the energy was 40W and the time was 5 min. All animals were sacrificed one week later, and the size of coagulative necrosis was measured, then the specimens were studied histopathologically. The liver function was also examined before, immediately and one week after PMCT. Results The coagulative necrosis was whitish-slight pink in color on gross appearance and elliptical in shape. The long diameter was (30.08±5.83)mm, with (14.75±3.66)mm of the short one. Histopathologically, swollen of the hepatocytes, degenerative changes and coagulative necrosis were observed in the liver specimens, meanwhile combined with inflammatory reactions and hyperplasia of the fibrotic tissue, as well as microabscess formation, hemarrhage and macrophage reaction. No definite change of the liver function was noted before and after PMCT procedure. Conclusion PMCT could effectively lead to coagulative necrosis of the liver tissue, and provide a new method for treating liver tumors.
