| 目的 重新评价传统食管癌CT分期,进一步制定合理的T分期。方法 102例食管癌患者行术前胸部CT常规扫描,其中45例行增强扫描。结果 CT扫描资料按照Moss分期标准分期,与临床分期对照,二者一致性较差(Kappa=0.27);按Tio分期标准对局部病变分期与术后病理T分级对照一致性较好(Kappa=0.46)。我们推荐改良T分期标准,经统计检验与术后病理T分级有较好一致性(Kappa=0.45)。结论 改良T分期标准与PTNM分期标准一致性好;且判断主动脉受侵标准准确率高。 |

| Objective To reevaluate the accuracy of conventional CT staging criteria with EC, and make a reasonable staging of the disease. Methods CT scan was performed in 102 patients before surgery. Of whom enhanced CT scan was in 45. Results The preoperative CT scan materials of 102 patients analyzed according to Moss CT criteria were not well correlated (kappa=0.27) with clinical pathology staging. The data were also analyzed based on the criteria suggested by Tio in 1989, and the Results significantly correlated with those from pT classification (Kappa=0.46). We modified the T classification which was classified into four stages, the data obtained according to the criteria were significantly associated with those based on the pT classification (Kappa=0.45). Conclusion Modified T criteria made the imaging diagnosis accordance with pathological diagnosis. The diagnostic accuracy of aorta invasion was improved with modified T staging criteria. |
