目的 探讨螺旋CT肝动脉期扫描及三维重建对原发性肝癌肝动脉供血血管及血供情况诊断的临床应用价值。方法 84例原发性肝癌行螺旋CT肝动脉期扫描,其中16例行肝动脉系统三维重建,三维重建方法包括最大强度投影(MIP)和表面遮盖成像(SSD)。结果 2DCT 75例可见供血肝动脉影伸入病灶内,分为6个类型:中央单支直条形(17例),中央单支树枝型(26例),中央多支型(13例),周围单支型(5例),周围多支型(9例),混合型(5例)。9例病灶未见明确供血肝动脉血管。肝动脉期血供分为多血供型(59例)及少血供型(25例)。16例肝动脉系统血管MIP及SSD重建,显示腹腔动脉,脾动脉,肝总动脉,肝固有动脉、肝左右动脉、肿瘤肝动脉供血血管及肿瘤血管形态。结论 螺旋CT肝动脉期及肝动脉系统血管三维重建,可以初步了解原发性肝癌肝动脉供血血管及肝动脉血供丰富情况,对指导肝癌介入治疗插管、病人选择及治疗效果的预测将有一定的临床价值。 |

Purpose To dicuss the clinical application of hepatic helical artery phase CT scanning and 3DCT in evaluating the supply vessels and blood supply of liver cancer. MethodsEighty four patients with primary liver cancer were performed by hepatic helical artery phase, 16 of which MIP and SSD of hepatic artery system were used as three dimensional models. Results2DCT demonstrated the supply vessels in 75 cases, divided six types:center single vessel (n=17), center single tree branch vessels(n=26), center poly vessels(n=13), periphery single vessel (n=5), periphery poly vessels(n=9) and mix(n=5). Supply vessels could not be shown in 9 cases. According to the number and shape of supply vessels and intertumor enhanced vessels, the blood supply of primary liver cancer was divided two types: more blood supply(n=59) and less blood supply(n=25). MIP and SSD of liver artery system (n=16) could demonstrate the celiac artery, common hepatic artery, proper hepatic artery, left and righ main hepatic artery, supply vessels and intertumor enhanced vessels. ConclusionHepatic helical artery phase CT scanning and 3DCT could demonstrate the supply vessels and blood supply of liver cancer, so it has an important value for planning interventional treatment. |
