      目的 评价LeVeen电极经皮肺穿刺射频消融治疗肺部肿瘤的可行性与安全性,分析治疗前后CT变化特征。方法 常规局麻,CT引导下采用LeVeen电极针对67例肺部肿瘤进行单治疗点或多点重叠的射频消融治疗。结果 67例中59例顺利完成了治疗,主要临床表现有穿刺点及胸部的不适感、胀痛感以及发热、出汗。CT见病灶周围有水肿(43),病灶增大(37),病灶中心出现点状低密度区(38),穿刺道的出血(6),少量气胸(13),仅1例出现张力性气胸。术后3天以内有低烧,咳嗽。随访复查3个月后病灶不同程度缩小。结论 射频消融治疗肺部肿瘤安全、有效,严格地选择病例可以达到非手术切除的目的。病灶周围的水肿、病灶体积增大,以及病灶中心出现点状低密度区是治疗后CT表现。

      Objective To evaluate the efficacy of CT guided percutaneous radiofrequency ablation(RFA) of lung malignancies and the CT features after treatment. Methods Sixty-seven patients with pulmonary malignancies were treated with CT guided percutaneous RFA. Local anesthesia was achieved with intradermal and subcutaneous lidocaine. LeVeen needle electrode and RF 2000 radiofrequency generator were used. A single-point or multipoint overlapping technique was selected according to the size of the tumor. Results Fifty-nine of 67 patients had successfully finished procedures. The main complains during the treatment were malaise of the puncture point and the chest. Sweat was a common manifestation. Fever and cough also were found 3 or 4 days after treatment. The CT features were edema at the circumference of the lesion(43 patients), enlargement of the lesions(37 patients), heterogeneous attenuation area in the malignancy(38 patients), hemorrhage of the puncture path(6 patients) and the pneumothorax(13). Usually the pneumothorax was very small that did not warrant intervention. Only one patient presented tension pneumothorax during the procedure. After being closed drained, the patient finished the treatment. Conclusion CT guided percutaneous radiofrequency ablation is an innovative and minimally invasive technique for the treatment of lung malignancies. The efficacy of this procedure can be assessed by the CT features and the alteration of the impedance. Edema at the circumference of the lesion, enlargement of the lesions and heterogeneous attenuation area in the malignancy are the signs of effective treatment.
