| 目的 探讨超声引导下局部治疗结合门静脉化疗在中晚期肝癌中的应用。方法 超声引导下局部治疗20例(26灶):15灶(直径<3cm)行微波治疗;11灶(直径>3cm)行无水酒精注射治疗。同时行门静脉化疗。全部病灶治疗后穿刺活检或随访。结果 随访时间3~25个月,一年生存率100%,两年生存率90%。微波治疗组均经活检证实完全性坏死;无水酒精注射治疗组US及 CT报告病灶稳定。门静脉化疗预防组无癌栓出现;治疗组癌栓3例缩小,2例消失。结论 中晚期肝癌采取局部治疗结合门静脉化疗对预防肝癌转移,延迟复发及改善预后有肯定意义,明显延长了生存期。 |

| Objective To investigate the application of local treatment combining with chemotherapy via portal vein guided by US in advanced hepatic cancer. Methods 26 lesions in 20 cases of hepatic cancer were given local treatment guided by US.15 lesions(lesion' diameter<3.0cm) were given treatment of microwave coagulation. 11 lesions (diameter>3.0cm) were given treatment of injecting absolute alcohol. Meanwhile 20 cases were given chemotherapy via portal vein guided by US All cases were following-up examination or biopsy. Results The time of following-up are between 3-25 months. Rate of survival of first year is 100%, that of the second year is 90%. The complete necrosis was reported by biopsy in the group of microwave coagulation. Data of following-exam of injected absolute alcohol by US and CT show: the sizes of lesion didn't develop. After chemotherapy via portal vein , tumor thrombosis didn't appeared in preventive group, it disappeared in 2 cases and shrank in 3 cases of treating group. Conclusion Local treatment combining with chemotherapy via portal vein is of certain value in preventing metastasis and relapse, improving prognosis and prolonging the period of survival to advanced hepatic cancer. |
