| 目的 探讨听骨链螺旋CT仿真内镜(CTVE)成像的最佳观察阈值。方法 对20例健康志愿者40耳和单纯型慢性化脓性中耳炎患者23耳分别进行了听骨链的 CTVE 观察,比较了不同的观察阈值对听骨链结构的显示能力。制订了图像质量评分标准作为比较的半定量指标。结果 正常组观察阈值上限为 -550HU~-450HU 时图像质量评分较高,而中耳炎组观察阈值上限为 +200HU~+300HU 时图像质量评分较高。CTVE 图像上听骨链的外形随观察阈值上限的不同而变化。观察阈值上限较低时出现很多漂浮状伪影;随着观察阈值上限的升高,镫骨和 / 或砧骨长脚及其砧镫关节首先逐渐变小、变细乃至消失。结论 在临床工作中,正常和病变组的观察阈值宜分别在选择为 -524HU 和 +250HU 后再根据所要观察的部分作适度调整。 |

| Purpose To study optimal threshold value of helical CT virtural endoscpy (CTVE) in auditory ossicular chain. Methods CTVE of auditory ossicular chain were performed in 40 ears of healthy volunteers and 23 ears with simple type chronic otitis media.The ability to show auditory ossicular chain were compared among various threshold values.The score criterion was worked out as a semiquantitative index to assess image quality. Results The auditory ossicular chains were demonstrate better when -550HU to -450HU and +200HU to +300HU was selected respectively as the higher threshold value in the normal group and the otitis group.Floating artifacts were viewed when the threshold value was lower.When the threshold value was raised, the stapes ,and the long crus of the incus ,and the incudostapedial joint was diminished,and thin, and even disappeared. Conclusion In clinical practices, the threshold value of the normal group and the otitis group is selected respectively -524HU and +250HU, then adjusted suitablity by the observed portion of auditory ossicular chains. |
