| 目的 探讨超声、核素及实验室检查对桥本病的诊断价值。方法 本组资料31例,男6例,女25例,年龄23~75岁,平均44.55±11.67岁,病程半年~14年。全部为我院1996年3月至1999年12月的住院病人,经手术或穿刺病理组织学证实为桥本病;其中20例因甲状腺结节性质待定收入院,2例为结节性甲状腺肿合并桥本。根据甲状腺肿大特征分为单纯肿大型(11例)和结节肿大型(20例)。Tg-Ab或／和Tm-Ab升高者23例。结果 两组桥本病患者甲状腺均较正常组肿大(P<0.001)。超声、核素显像均有特异性改变。与正常图像相比,超声的敏感性为100%(31/31),核素为93.5%(29/31)。特异性超声为77.4%(24/31),核素不能明确提示为桥本病;Tg-Ab或／和Tm-Ab升高者为88.5%(23/26)。若将三种检查结合起来,则符合率为87.1%(27/31)。结论 桥本病的超声、核素影像各有其独特性。在判断桥本病中,超声的特异性优于核素,若结合实验室检查可提高诊断符合率。 |

| Purpose To evaluation the diagnostic value of ultrasonography and scintigraphy in the hashimoto thyroiditis. Methods The group data include 31 cases, diagnosed to be the hashimoto thyroiditis by surgery or biopsy pathohistology. According to thyroid enlargement characteristic, the cases were classified single enlargement(11 cases) and nodule enlargement(20 cases). 23 cases associate with Tg-Ab or/and Tg-Ab highness. Results Thyroid gland of the cases of two group hashimoto thyroiditis enlarge compared with normal group(P<0.001). The both of ultrosonogrphy and scintigraphy have specific characteristics. Compared with normal image, the sensitivity of ultrosonogrphy is 100%(31/31) and scintigraphy is 93.5%(29/31).The specificity of ultrosonogrphy is 77.4%(24/31) and scintigraphy can not diagnose hashimoto thyroiditis definitely. The patients with enhanceed Tg-Ab or／and Tm-Ab are 88.5%(23/26). If the three examinations are combined, The accurate diagnose rate is 87.1%(27/31). MConclusion Ultrasonography has itself specificity, either scintigraphy. In estimate hashimoto thyroiditis,the specificity of ultrosonogrphy excel scintigraphy. If laboratory examination is combined, the accurate diagnose rate is enhanced. |
