目的 用磁共振质子波谱(MRS)的方法研究阿尔茨海默病(AD)额叶代谢的改变。 方法 在2.0T的场强条件下,对15例阿尔茨海默病和15名正常老年人的额叶进行磁共振质子波谱(采用STEAM序列)采集。分别测量质子波谱中所观察到的代谢产物的峰高,以肌酸和磷酸肌酸的峰高为参照计算峰高的比值,进行相对定量分析。并对代谢产物的变化与临床简易精神测量评分(MMSE)之间和与海马结构体积测量值之间进行相关性分析。 结果 阿尔茨海默病前额叶的NAA/Cr比值较正常对照组降低(P<0.01),mI/Cr的比值较正常对照组升高(P<0.01),Cho/Cr无变化。AD病人额叶NAA/Cr的降低与MMSE评分和海马体积呈正相关,mI/Cr的升高与MMSE评分和海马体积改变呈负相关。 结论 磁共振质子波谱能无创性检查阿尔茨海默病脑部的代谢变化,揭示其部分病理生理改变,1H-MRS所显示的阿尔茨海默病的NAA降低和mI升高,能提高中早期阿尔茨海默病的诊断正确率。 |

Purpose To study the metabolic alterations of probable Alzheimer disease with proton magnetic resonance spectroscopy for better understanding the pathophysiology of Alzheimer disease. Methods STEAM 1H-MRS were performed in the frontal lobe in 15 patients with probable Alzheimer disease and in 15 healthy control on 2.0T MRI/MRS system. The height of resonance peak of different metabolites was measured and the ratios of the other metabolites to Cr were calculated. We also studied the correlation between the ratio of NAA/Cr, mI/Cr and MMSE (mini-mental-state examination), and the volume of hippocampal formation. Results on patients with probable AD, there was a significant reduction in the metabolite ratios NAA/Cr in frontal lobe cohort compared with the control group. In addition, the AD group showed a significant elevation in the mI/Cr ratio in frontal lobe cohort compared with the control group. There were no significantly differences of the ratio Cho/Cr between healthy control and patients with probable AD. Significant correlation between NAA/Cr, mI/Cr and MMSE, and the volume of hippocampal formation with probable AD. Conclusion 1H-MRS could provide biochemical and metabolic information for the pathophysiology investigation on patients with AD in vivo noninvasively. The combination of high mI and low NAA at examination with 1H-MRS shows promise as an early diagnostic test for AD. |
