| 目的 探讨肝细胞癌的螺旋CT征象与病理及TNM分期间的关系。方法 分析螺旋CT平扫及增强检出的肝细胞癌31例,其中13例经CT导向下穿刺活检证实,18例经手术病理证实。结果 ①螺旋CT显示瘤体直径＞3cm、瘤体多发、边缘模糊不伴晕圈征或中心伴坏死的病灶,其组织学分级均明显高于瘤体单发、边缘清晰伴晕圈征、中心不伴坏死者(P<0.05)。②在TNM分期方面Ⅲ+Ⅳ期的组织学分级均明显高于Ⅰ+Ⅱ期(P<0.05)。结论 ①SCT表现为瘤体直径>3cm、多发、瘤体边缘模糊及瘤体中心伴有坏死者的组织学分化程度差且肿瘤侵袭力强;②螺旋CT对肝细胞癌的TNM分期能较准确地反映其侵袭力。 |

| Objective The Purpose is to investigate the correlation among SCT features and pathology with hepatocellular carcinoma(HCC) TNM staging. Methods 31 cases of HCC observed by plain SCT and enhanced SCT ereanalysed,13 in 31 cases were identified by SCT-guided percutaneous biopsy,and 18 in 31 cases were identified by pathology. Results ①The histological grades in tumor were more than 3cm in diameter,multi tumor,blurred contous with halo sign, and central necrosis were higher than those in tumor with less than demarcated and nonecrosis(P<0.05).②The histological grades in Ⅲ+Ⅳ staging of HCC were higher than those in Ⅰ+Ⅱ TNM staging of HCC(P<0.05). Conclusion ①HCC of multinodules,blurred contours and central necrosis showed SCT reflectded poor histological differentiations.②The TNM staging of HCC provided by SCT accurately reflected the aggessiveness of the tumor. |
