      目的探讨不同浓度对比剂直接法螺旋CT 胆道造影对胆管解剖的显示效果及胆系疾病的诊断价值。 方法对11 例阻塞性黄疸和10 例胆系术后患者,通过经皮肝穿胆道引流管( PTCD) 或胆系术后留置的引流管注入 1 %、5 %、10 %三种浓度的泛影葡胺进行直接法螺旋CT 胆道造影检查,并进行最大密度投影(MIP) 和表面遮盖 (SSD) 重建,多层面和多角度观察胆管解剖。结果5 %浓度组21 例病人共显示尾叶胆管67 支,平均3.19 ±0.81 , 其中15 例清晰显示出三级以上胆管,明显高于1 %和10 %浓度组。结论直接法螺旋CT 胆道造影以5 %的泛影葡 胺效果最佳,是诊断胆系疾病的可靠方法。

      Objective To evaluate the value of direct injection helical CT cholangiography using contrast agents with different concentrations for depicting the biliary anotomy and diagnosing the biliary diseases. Methods Direct injection helical CT cholangiography was performed via biliary drainage tube of percutaneous transhepatic cholangiography drainage (PTCD) or postoperatively perserved tube with 1 % ,5 % and 10 % meglumine diatrizoate respectively in 11 cases with biliary obstrction and 10 cases after biliary tract operation. Maximum intensity projection (MIP) and shaded surface display(SSD) were applied to observe the biliary anotomy with multislice and arbitrary angle modes. Results 5 % group in 21 cases showed 67 branches of bile ducts of caudate lobe (mean value 3. 19 ±0. 81) , and larger than the outer third of the hepatic parenchyma were clearly visualized in 15 of 21 cases. The presence of the branches of the biliary ducts in 5 % group was obviously more than that in 1 % group or 10 % group. Conclusion Direct injection helical CT cholangiography is a reliable method for depiction of the biliary branches and diagnosis of biliary disease.
