| 目的探讨彩超对肾恶性肿瘤的价值。方法用彩色多普勒血流图(CDFI) 观察不同病理类型、大小不
等的肾恶性肿瘤,测量血流速、流量并按特点分型。结果51 例肾恶性肿瘤中,多血管型33 例占64.7 % ,少血管型
10 例占19.6 %、极少血管型8 例占15.7 %。瘤体大小及血流特征为:透明细胞癌,瘤体(619～915cm) 内血管增多、
增粗呈分叉状或网状,流速60～104cm/ s ;颗粒细胞癌,瘤体大者5～7cm ,小者< 215cm ,血管少或极少,呈点状血流
束,流速50～80cm/ s ;肾母细胞瘤,瘤体( > 10cm) 内血管多扭曲呈蚯蚓或峰窝状,流速达160cm/ s ;肾盂癌,瘤体大者
3～5cm ,小者< 215cm ,血管少或极少,呈星点状时隐时现,流速20～70cm/ s。结论CDFI 可显示不同类型的肾肿
瘤,瘤体内血管数量及分布不同,CDFI 各项测值有助于肿瘤的分类诊断。 |

| Objective To evaluate the diagnostic value of CDFI to kidney malignant tumor. Methods The blood velocity and flux of different pathological types and sizes of kidney malignancy were measured. Results In 51 cases ,33 with
more blood vessels ,10 with a few blood vessels ,8 with rare blood vessel. There were increased number and diameter of
blood vessels ,which furcated or netted ,in clear cell carcinoma (6. 5～9. 5cm) . The blood velocity was about 60～104cm/ s
granulosa cell carcinoma ,no matter which were about 5～7 cm or smaller than 2. 5cm ,there were only a few blood vessels in
their bodies. The blood velocity was about 50～80 cm/ s ephroblastoma ,larger than 10 cm in size ,the blood vessels twisted
like earthworm or honeycomb ,the blood velocity was about 160cm/ s pyelitis carcinoma ,not larger than 5 cm ,there were a
few or non blood vessels in tumor bodies ,the velocity was about 20～70cm/ s. Conclusion The number and distribution of
blood vessels are variant in different types of kidney tumors. CDFI is helpful to the discriminating diagnosis of these tumor. |
