| 目的探讨左房M 型超声指标评价冠心病患者左室舒张功能的价值。方法获取左房M 型超声图像,
在左房排空早、晚期测算主动脉后壁运动斜率Eslope、Lslope 及左房缩短分数FSg、FSa ;采用频谱多普勒超声测取二
尖瓣口舒张期血流速度E、A 及E/ A 比值,测取左室等容舒张时间( IVRT) ;采用智能声学定量技术测取左室舒张
早、晚期及全舒张期充盈量( EDFV、LDFV、DFV) 。结果冠心病患者按E/ A 比值分为三小组,与正常对照组相比:
①E/ A ≤1 组患者IVRT 明显延长,FSg、Eslope、EDFV 显著减小,FSa、LDFV 显著增大,DFV 及Lslope 改变无统计学
意义; ②1 < E/ A < 2 组患者IVRT 显著延长, FSg、FSa、Eslope、Lslope、EDFV、LDFV 及DFV 均显著减小; ③E/ A ≥组患者各项测值均显著降低。结论左房M 型超声指标测量简单,较二尖瓣口多普勒血流频谱能更准确、更直接地
反映左室舒张功能的变化情况。 |

| Objective To assess the value of left atrial M-mode echocardiography parameters in evaluation left ventricular
diastolic function. Methods We determined the left atrial (LA) M-mode echograms and Doppler transmitral flow velocity
profiles in 40 patients with CAD and 30 normals. We calculated LA global and active fractional shortening and the slope of
both early and late displacement of the posterior aortic wall during LA emptying by M-mode echocardiography. We calculated peak velocity of early ( E) and atrial (A) filling ,E to A ratio ,and time of isovolumic relaxation ( IVRT) by pulsed Doppler
echocardiography. The filling volume of early and late and total diastole were measured by Acoustic Quantification (AQi) .
Results Patients with CAD were divided into three groups according to their E/ A ratio. Compared with normal subjects :
①abnormal relaxation as E/ A ratio ≤1 : IVRT were prolonged. FSg ,Eslope ,EDFV were decreased ,but FSa ,LDFV were
increased significantly. There was no obvious change in DFV ; ②“pseudonormalized”pattern as 1 < E/ A ratio < 2 ,FSg ,Esg ,
Eslope ,Lslope ,EDFV ,LDFV and DFV were reduced significantly. There was obvious prolongation in IVRT; ③restrictive filling as
E/ A ratio ≥2. All indexes of patients in this group were decreased significantly. Conclusion LA M-mode recordings provided a
more reliable and direct noninvasive assessment of diastolic function in CAD than mitral Doppler indexes. |
