| 目的研究老年人脑缺血性病变(SICVD) 的特点,分析SPECT 脑局部血流灌注显像(rCBF) 对SICVD 的
诊断价值。方法36 例SICVD 患者均接受rCBF 及脑MRI (部分行MRA) 检查,比较rCBF 与MRI 的检查结果。结
果①与MRI 相比,rCBF 更敏感地显示大脑各叶皮质的缺血性病变,较好地显示基底节核团的病变,对小脑、脑干
及皮层下白质、内、外囊病变显示不良。老年人颞叶缺血比较多见。②rCBF 因可敏感地显示枕叶病变而对椎基底动
脉系统缺血性病变(VBID) 有一定的诊断价值,与MRI 结合,为VBID 的诊断提供更多的客观依据。③部分患者存
在缺血性病变区域的相应血管病变。结论rCBF 与MRI 结合,可更全面反映SICVD。 |

| Objective To study the characteristics of senile patients with acute ischemic cerebral vascular disease
(SICVD) ,analysis the value of SPECT brain perfusion (rCBF) in the diagnosis of SICVD. Methods 36 SICVD patients
took the test of rCBF and cerebral MRI (a part of them did MRA) , the result were compared. Results ①Compared with
MRI ,on showing SICVD ,rCBF was more sensitive in the area of cerebral cortex ,sensitive in the basal ganglia ,unsensitive
in cerebellum , brainstem ,subcortex white substance and internal and external capsules. The SICVD in temperal lobe was
popular in senile patients. ②Combined with MRI ,rCBF help to improve the diagnosis of vertebral-basilar artery ischemia
(VBI) by reflecting occipital lobe SICVD sensitively. ③Corresponding cerebrovascular lesions were seen in the areas of
SICVD in some patients. Conclusion Correlation of rCBF and MRI could disclose the SICVD more completely. |
