| 目的 利用M 型及二维超声心动法评价重度妊高征( PIH) 患者分娩前后心脏形态变化。方法 重度PIH 患者32 例,正常妊娠(NP) 24 例,采用美国Acuson XP/ 10 彩色电脑声像仪,常规测量室壁厚度、各房室腔内径,注意有否心包积液。于产后2 个月内复查。结果 PIH 组左房收缩末期前后径、左室舒张末期前后径较NP 组明显增大(38.9 ±4.5mm vs 34.6 ±4.4mm , P < 0.01 ;51.2 ±5.8mm vs 47.1 ±4.2mm , P < 0.01) ,室间隔轻度增厚,PIH和NP 组心包积液发生率分别为31.3 %和16.7 %。25 例PIH 患者产后复查,左房和左室内径恢复到正常产后水平者各为70 %和47 %;3 例室间隔厚度大于12mm 者产后复查时2 例恢复至正常产后水平;80 %妊高征患者心包积液消失。结论 重度PIH 患者孕晚期心脏形态变化以左房室腔增大、室间隔增厚及心包积液为主。产后2 个月内,大多数患者心包积液消失,约半数患者其它心脏形态学异常可恢复至正常产后水平。 |

| Purpose The study was to evaluate the cardiac morphologic changes in pregnancy-induce hypertension (PIH)patients before and after parturition with echocardiography. Methods Thirty2two patients with severe PIH ,24 normalpregnant (NP) women were examined by echocardiogrpahy. The diameters of atria ,ventricles ,great arteries and wall thickness were measured with Acuson 128 XP/ 10 computed sonography. The pericardial effusion (PE) was also detected. Pregnant women were to be reexamined in two months after delivery with the same measurements. Results LA and LVd weresignificantly enlarged (38.9 ±4.5mm vs 34.6 ±4.4mm;51.2 ±5.8mm vs 47.1 ±4.2mm , P < 0.01) . IVS slightly thickened. The occurrence rates of PE were 31.3 % ,16.7 % in PIH and NP group respectively. In 25 reexamined PIH patients ,70 % ,47 % of LA and LVd returned to NP postpartum level. The thickened IVS( > 12mm) retumed to normal in 2 of 3PIH patients and the PE of 80 % PIH patients disappeared. Conclusion The most significant changes of the cardiac mor2phology are the increased diameters of LA ,LVd ,thickened IVS and PE. Most of PIH patients’PE disappeared and abouthalf of other abnormalities could recover to be the level of normal pregnant postpartum within 2 months. |
