| 目的 探讨肝包膜退缩征的CT 表现及其诊断价值。方法 回顾性分析经手术病理及临床综合诊断证
实的206 例肝肿瘤及肿瘤样病变的CT 表现,观察肿瘤或肿瘤样病变的部位、大孝形态、数目、密度、及内部情况,并
对其邻近的肝包膜以及与肝包膜有关的征象进行分析。结果 CT 像显示肝包膜退缩征24 例。根据肝包膜表面光滑与否分为两型: (1) 规则形,19 例。退缩的肝包膜表面光滑、整齐,呈反弓状或脐凹状。(2) 不规则形,5 例。退缩的肝包膜参差不齐,呈锯齿状。24 例肝包膜退缩征阳性者均见于恶性肝肿瘤,特异性100 %。结论 肝包膜退缩征是恶性肝肿瘤的一个特征性CT 表现,而良性肿瘤或肿瘤样病变却与此无缘。 |

| Purpose To evaluate the CT manifes-tation and dignostic value of capsular retraction in malignant hepatic tumors. Methods 206 patients with hepatic tumor or tumor- like proved by percutaneous biopsy ,surgery or clinical dignosrs
were retrospectrively analysed. Tumor or tumor- like location , size , shape ,number ,margin ,density ,and internal structure
were evaluated. Features relative to capsular retractron of live were noted. Results Of 206 patient with intrahepatic tumors or tumor-likes ,CT scans in 24 patient showed capsular retraction or umbilication. According to whether the surface of
capsular near the tumor or tumor-like was regular or not ,capsular retraction (CR) can be classified into two types : (1) Regular in 19 patients. This kind of CR was smooth and regular. (2) Irregular in 5. The tumor margin extending to the liver
surface was irregular. All 24 tumors were malignant . The specicity was 100 %. Conclusion Capsular retraction occurs the
hepatic surface adjacent to a variety of malignant hepatic tumors. This findings was never associated with a benign tumor or
tumor-like. |
