目的 探讨高分辨率CT(HRCT)及三维(3D)重建技术对视神经管骨折的临床应用价值。方法 对外伤
性视神经损伤者共40例行视神经管高分辨率CT扫描及三维重建,并对骨折的CT表现进行分析。30例骨折行视
神经管减压术。结果 40 例中39 只眼视神经管骨折,内壁骨折29只眼,占74.4% ,下壁骨折16只眼,占41.8%。
视神经管骨折可分为五型: ①凹陷型; ②线状型; ③粉碎型; ④嵌入型; ⑤混合型。本组40例中36例有眶壁骨折,占
90%。结论 高分辨率CT扫描及3D重建可提高对视神经管骨折的诊断水平,为临床选择治疗方案提供可靠的影
像诊断依据。 |

Objective To evaluate the clinical application of HRCT and 3D reconstruction in the fracture of optic canal.
Methods HRCT and 3D reconstruction were performed in the optic nerve injury of 40 cases ,30 cases were proved by optic
canal decompression. Results 39 fractures were observed in 40 cases. Medial wall of optic canal was the most common position with 29 fractures (74. 4 %) ,16 fractures (41. 0 %) were in inferior wall. Optic nerves were obviously compressed 25
cases(64. 1 %) . The fracture were divided five types on CT : ①concave ; ②lineal ; ③comminuted ; ④inlay ; ⑤mixed. 36 cases
had orbital wall fractures in 40 cases. Conclusion Conventional CT can no longer meet the needs of diagnosis of fracture of
optic canal. HRCT and 3D reconstraction can improve diagnostic ability ,provide reliable imaging information for selecting
treatment plan. |
