目的 研究颅中窝海绵状血管瘤的MR 影像表现,并与脑膜瘤进行了对比分析。方法 收集经手术和病理证实的颅中窝海绵状血管瘤33 例以及颅中窝脑膜瘤96 例。使用Windows 8.0 版社会科学统计软件包对两组肿瘤的MR 及临床观察指标进行单因素和多因素分析,统计学方法包括χ2检验和Logistic回归。结果 χ2 检验显示患者性别、肿瘤大孝肿瘤形态、肿瘤实质的均匀程度、T1WI 信号强度、T2WI 信号强度、肿瘤增强均匀性等7个因素在海绵状血管瘤组与脑膜瘤组之间存在着差异, P<0.01 ;肿瘤边缘和有无脑膜增强2个因素在两组间也存在差异, P<0.05 。Logistic 回归方程中,变量“T2 加权像信号强度”的P 值为0.0005 , P < 0.01 。结论 本组资料分析表
明,在颅中窝海绵状血管瘤和脑膜瘤的MR 影像鉴别诊断诸多因素中, T2WI 上肿瘤表现为高信号影最有意义。虽然少数脑膜瘤也可以有长T2 信号表现,但此征象仍强烈提示海绵状血管瘤的可能性。结合其他因素的分析将有助于提高颅中窝海绵状血管瘤的MR 影像诊断水平。 |

Objective To study the MR imaging appearances in a series of cavernous angiomas in the middle cranial fossa
and compare these findings with the MR imaging features of meningiomas in the middle cranial fossa. Methods A retrospective analysis was performed of MR examinations in 33 patients with histologically proved cavernous angiomas in the
middle cranial fossa ,these were then compared with imaging findings in 96 cases of meningiomas histologically proved. The
data were evaluated with a statistical analysis system (windows 8.0 statistics package for social science) usingχ2 test and
Logistic Regression. Results Significant difference ( P < 0.01) in sex , size of tumor ,shape of tumor ,homogenous degree
inside tumor ,T1WI signal intensity , T2WI signal intensity and homogenous degeree of enhancement were found between
cavernous angioma group and meningioma group. In addition , there was a signifcant difference ( P < 0.05) between two
groups with margin of the tumor and enhancement of the meningeal. In logistic regression analysis , T2WI signal intensity
shows significant difference ( P < 0.01) between two groups. A trend ( not statistically significant in Logistic regression
analysis) was noted for absence of“tail sign”and only 15.2 %of male in cavernous angioma group. Conclusions Our data
indicate that high signal intense characteristics of the tumor on T2WI appears to be a sensitive and specific marker for differentiating cavernous angioma from meningioma in the middle cranial fossa , with a very small overlap between the two
groups. Cavernous angioma can be strongly suspected if MR images reflect the above features. |
