      目的 比较分析MRI 与核素骨扫描(BS) 在前列腺癌骨转移诊断方面的敏感性、特异性与准确性。方法 71 例经病理证实的前列腺癌,在治疗前分别行核素骨扫描和盆腔MRI 检查。比较两种方法对盆腔范围内骨转移 显示情况。结果 71 例前列腺癌患者,以盆腔范围为观察目标,确诊为骨转移瘤者19 例。MRI 正确诊断18 例,假 阴性1 例,无假阳性,其诊断敏感性为9417 % ,特异性为100 % ,正确诊断率为9816 %;核素骨扫描正确诊断17 例,假 阴性2 例,假阳性8 例,其诊断敏感性为8915 % ,特异性8416 % ,正确诊断率8519 %。以病灶为统计单位,在盆腔范 围内,共92 个病灶确诊为转移;MRI 诊断正确91 个病灶,假阴性1 个;核素骨扫描诊断正确66 个病灶,假阴性26 个,假阳性18 个。敏感性MRI 为9819 % ,而核素骨扫描为7117 %。结论 在显示骨转移瘤方面,MRI 较核素骨扫 描具有更高的敏感性、特异性与准确性,但受扫描范围限制。
英文摘要:
      Objective To evaluate the ability of bone scintigraphy (BS) and MRI in the demonstration of bone metastases of prostatic cancer and their sensitivity ,specificity and accuracy. Methods 71 patients with prostatic cancer ,before therapy ,were imaged with BS of the whole body and pelvic MRI. All the cases were proved by histopathology. Compare the demonstration of bone metastases with the two imaging modalities. Results In the pelvic region (from L3 to the head of femur) ,19 of the 71 patients with prostatic cancer were proved to have bone metastases. With MRI ,the accurate diagnosis were given in 18 patients ,1 given false negative diagnosis ,and no false positive diagnosis. As result ,the diagnostic sensitivity is 9417 % ,the diagnostic specificity is 100 % ,the accuracy is 98. 6 %. With BS ,the accurate diagnosis were given in 17 patients ,2 given false negative diagnosis ,and 8 false positive diagnosis. As result ,the diagnostic sensitivity is 89. 5 % ,the diagnostic specificity is 84. 6 % ,the accuracy is 85. 9 %. If take the total lesions as the samples ,92 lesions were demonstrated as metastatic lesions. In MR imaging ,true positive lesions were 91 ,false negative lesions were one. In Bone Scintigraphy ,true positive lesions were 66 ,false negative lesions were 26 ,and false positive lesions were 18. The sensitivity of MRI was 9819 %. The sensitivity of BS was 7117 %. Conclusion In diagnosing bone metastases ,MRI is more sensitive and correct than BS.
