目的 研究颅内毛细血管扩张症( ICT) 的MR 及临床表现,评价梯度回波序列在ICT 诊断中的应用价
值。方法 30 例诊断为ICT 的患者MR 扫描包括自旋回波(SE) T1 加权像、快速自旋回波T2 加权像( TSE) 及梯度
回波T2 加权像( T2 FFE) 。并对其MR 表现进行分析。结果 ICT 可见于脑桥、小脑、基底节区、丘脑及皮层下。其
中5 例于自旋回波T1 加权像上呈稍低信号,1 例于快速自旋回波T2 加权像上呈稍高信号,其余均呈等信号强度。
而在T2 加权梯度回波序列上所有病例均呈明显低信号。30 例患者存在一系列临床症状,但仅3 例患者的临床症状
可以肯定于ICT 有关。结论 T2 加权梯度回波序列是检出ICT 的敏感方法且在ICT 的诊断及鉴别诊断中具有关
键性作用。少数患者可因ICT 或ICT 合并少量急性期出血而引起临床症状。 |

Objective To report clinical and MR features of intracerebral capillary telangiectasia ( ICT) and evaluate the
diagnostic value of MR. Methods 30 patients with ICT were included in this study ,all patients had undergone MR scan
with spin echo (SE) T2weighted ,turbo spin echo ( TSE) T2weighted and T2weighted fast field echo ( T2 FFE) sequence.
All the MR images were analysised. Results ICT was detected in pons、cerebellum、basal ganglia、thalamus and subcortex.
Five of the patients were slightly hypointense on SE T2WI ( n = 5) ,one was slightly hyperintense on TSE T2WI ( n = 1) ,
24 patients showed isointense on both T1WI or T2WI. All of the patients were showed marked signal loss on T2 FFE images. The patients presented avariety of symptoms but only three patients had symptoms which caused by ICT. Conclusion
T2 FFE sequence is more sensitive in detect ICT and is key to establish the diagnosis and differential diagnosis of ICT. A
few of such patients may presented clinical symptoms which caused by ICT complicate acute hemorrhage. |
