目的 探讨应用冠脉血流多普勒显像技术(CDCFI) 在检测犬心肌内血流方面的价值,观察声学造影剂和
冠脉扩张对心肌内血流的多普勒增强效应。方法 应用CDCFI 检测5 条开胸杂种犬心脏各节段心肌内血流信号;经主动脉根部注入Albunex 及短暂结扎并放开前降支后观察心肌内血流信号的改变;处死犬后沿超声所见较明显冠脉血流切面解剖以证实超声所见。结果 5 条犬各段心肌内均可显示散在彩色血流信号,多普勒频谱显示这些血流以舒张期灌注为主,并为前向血流,收缩期可见少量逆流。在这些节段中前壁、前间壁以及前侧壁血流最易显示,这些信号由前降支垂直发出,与前降支血流呈“T”字形分布;另外,4 条犬的前壁内可以显示1~2 条长条形而另1 条犬的前壁内可见许多条形血流信号,这些血流信号由心外膜冠脉垂直发出后,经心肌直接进入乳头迹4 条犬的室间
隔动脉血流信号由左冠脉主干发出,而另1 条犬由左前降支起始段发出,然后沿前室间隔和中室间隔朝向右室面行
程,并终止于室间隔右室面。这一点超声所见与解剖检查结果一致。进行声学造影后心肌内即刻充满丰富彩色信
号,较基础状态时明显增多且较明亮。短暂闭塞前降支冠脉并松开后,心肌内血流信号亦见明显增多。结论 CDC-FI 能敏感准确显示心肌内血流信号,为评价心肌血流灌注和分析心肌血流动力学提供新的直接依据。 |

Objective The study was performed to probe into the value of color Doppler coronary flow imaging system (CDCFI) in the examination of canine intramyocardial flow the in the assessment of flow Doppler enhancement after infu-sion of contrast agents and during coronary dilation. Methods Baseline intramyocardial flow in different segments of my-ocardium in 5 open-chest mongrel dogs was examined by CDCFI and by pulse-wave Doppler. Then Albunex was injected
through the aortic root and intramyocardial flow was observed sequently. Left descending artery was occluded for 20 seconds
and then relaxed to determined the change of intramyocardial flow in anterioseptal and anterior wall of left ventricle. At the
end of the experiment ,Dogs were killed and the hearts were incised to prove the findings by CDCFI. Results In every seg-ment of myocardium color flow signals could be found. The flow pattern was predominantly diastolic with systolic reversal.
Flow signals could be displayed most easily in anterior and anterioseptal wall. The intramyocardial coronary flow originated
from the related epicardial coronary and went into the myocardium vertically. So the epicardial and intramyocardial flow dis-tributed“T”2 like. 1 to 2 long-line flow signals in 4 dogs and many in 1 dog were found entering the myocardium and reach
directly papillary muscle. In septum a long line signal went from the origin to the right ventricular endocardium. This finding corresponded to the anatomic examination. The signals increased after the infusion of Albunex and in the condition of hyper-emia. Conclusion CDCFI could detect intramyocardial flow accurately and could provide direct hemodynamic information. |
