| 目的 评价B超及多普勒超声对胡桃夹现象的诊断价值。方法 应用彩色及脉冲多普勒研究3 例成人胡桃夹现象病例的左肾静脉(LRV)彩色血流及频谱图,通过简化柏努力方程计算LRV 与下腔静脉间的压力差。并与30 例随机选择的近肾端LRV 内径与腹主动脉和肠系膜上动脉间的LRV 前后径比值≥3 的病例作为对照组进行对比研究。结果 胡桃夹现象病例彩色多普勒显示LRV 受压狭窄处的血流加速,呈射流向下腔静脉内喷射；脉冲多普勒测得射流束的流速为102 ～142 cm/s,LRV 和下腔静脉间的压力差ΔP为3.5～6.6mmHg.对照组ΔP< 1m m Hg.结论 多普勒超声对胡桃夹现象的诊断明显优于普通B超,ΔP> 3.0mmHg 可作为诊断的标准。 |

| Objective To evaluate B mode and Doppler ultrasound in diagnosis of nutcracker phenomnon.Methods The blood flow of left renal veins(LRV) of three adult cases suffering from nutcracker phenomnon were studied with color and pulse Doppler ultrasound.The difference of pressure detween LRV and inferior vena cava(IVC) was calculated with simplified Bernoullis equation.The 30 cases selected randomly,all of them conformed to the ratio≥3 of the internal diameter of LRV proximal to the left kidney to the antero posterior diameter between the abdominal aorta and superior mesentery artery,as a contrast group were studied comprartively.Results The color Doppler ultrasound in the cases of nutcracker phenomnon showed that blood flow at the pressed and narrow portion of LRV was acceleraed and the boold was jetted into the IVC in jet flow mode.The velocity of the jet flow beam measured by the pulse Doppler was 102～142 cm/sec.The difference of pressure between LRV and IVC was ΔP=3.5～6.6 mmHg,that the contrast group was ΔP<1 mmHg.Conclusion The diagnosis with Dopple ultrasound for nutcracker phenomnon was obviously superior to that with B mode ultrasound.ΔP>3 0 mmHg could be regarded as a diagnostic standard. |
