      目的 本文回顾性地分析了50 例经手术后病理证实的脑海绵状血管瘤病例,结合临床及影像学表现进行综合分析,以求更好地指导选择恰当的治疗方法。方法 搜集1995 ~1998 年三年间脑实质内海绵状血管瘤手术后病例,男性28 例,女性22 例,全部病例均行MR 和/ 或CT 扫描,部分病例于平扫后增强扫描,所有影像资料进行综合分析。结果 脑实质内海绵状血管瘤影像学表现如下:①可发生于任何部位;②多发病灶病例占24%,单发病例76% ;③CT平扫为稍高密度影,部分病例可见钙化灶,注药后大都无强化;④MR 主要表现为混杂信号,周围可见短T2 低信号;⑤经手术后追踪发现,被手术病灶大都明显变小,部分病灶见软化灶,临床症状减轻或消失。结论 ①MR 对脑实质内海绵状血管瘤的检出率优于CT 检查,且可明确诊断;②病灶大而部位浅且出现明显相关临床症状可考虑手术治疗,病灶小而深在,临床症状隐匿者,及伴有癫痫发作者可在抗癫痫治疗的同时随访观察。

      Purpose The CT and MR imaging features of the cerebral cavernous angiomas are reviewed,based on analysis of fifty cases.In order that to direct clinical treatment as selected the methods that treated by surgical or others.Methods Collected the cases between 1995~1998 three years cerebral cavernous angiomas of our institute. All of them are post surgical cases, confirmed by pathology. 28 cases are male,22 cases female,CT and MR images were studied,all of them were examed by MR and /or CT scan,included precontrast and a part of them are postcontrast,all material have analized.Results The imaging apperance of cerebral cavenous angiomas have characters followed by:① No special location in brain, and multiple lesion in one case is 24% of all cases,and single lesion is 76%,slightly hyperdense on noenhhanced CT scan,some of them have calcification,After contrast administration,most of them are not enhanced.②MR apperances is typically,mulberry like lesion with a well delineated complex high and low intensity signal,around of lesion short T2 signal can be seen,espcially on gradient echo.③Postoperation,most of the lesion are decreased or disapper,some of them became encephalomalacic areas,are no clinical sysptoms apperance.Conclusion ①for cerebral cavernous angiomas can be dignosed by CT and MR imaging fetures and MR scan is advantage than CT scan;②According to the imaging fetures as size location and clinical apperance we can selected the different methods that may be suitability and include surgical and other nonoperation to treat the different lesion.
